【摘要】 目的　探讨高强度聚焦超声治疗未“开窗”肝右叶肝癌的临床疗效。方法　利用HIFU技术治疗未“开窗”的肝右叶肝癌 76例。比较患者HIFU治疗前后临床症状 ,实验室检查及影像学检查结果。结果　 76例患者临床症状较术前有明显改善 ,AFP较术前明显降低 (P =0 .0 0 5 )。CT、MRI复查肿瘤全部坏死的有 2 1例 ( 2 7.6% ) ,坏死区大于75 %的有 19例 ( 2 5 % ) ,坏死区大于 5 0 %的有 14例 ( 18.4% ) ,坏死区介于 3 0 %～ 5 0 %之间的 2 2例 ( 2 8.9% )。仅少数患者出现轻度烧伤 ,未观察到严重并发症发生。结论　HIFU作为一种新的肝癌局部治疗手段治疗未“开窗”的肝右叶肝癌同样可取得令人满意的疗效更多还原

【Abstract】 Objective To evaluate the clinical efficacy of extracorporeal high intensity focused ultrasound(HIFU) in the treatment of patients with hepatocarcinoma in the right lobe.Methods Seventy-six patients with hepatocellular carcinoma in the right lobe were treated by HIFU without open rib window. The clinical symptoms were observed, laboratory and follow-up imaging examinations were performed before and after the treatment.Results After treatment ,the symptoms were obviously relieved, Serum alpha-fetoprotein( AFP ) significantly decreased(P=0.005). Computed tomography (CT) and Magnetic resonance imaging (MRI) examinations showed complete coagulative necrosis of the tumer in 21 patients (27.6%), in 19 patients, the region of necrosis in the tumer was more than 75% in 14 patients(18.4%),the region was more than 50%, and the region was between 30% and 50% in 24 patients(28.9%).Conclusion HIFU provides a new, effective ,noninvasive method in treatment of patients with hepatocarcinoma.更多还原
