【摘要】 目的探讨下颈椎骨折脱位围手术期合并症、并发症及其治疗策略。方法总结伴急性颈脊髓损伤的下颈椎骨折脱位45例患者临床资料,记录、分析围手术期合并症和并发症及其治疗选择。结果呼吸衰竭死亡2例,其他患者均安全渡过围手术期。围手术期呼吸功能障碍15例,均接受气管切开。呼吸机辅助呼吸4例,肺部感染4例。术后局部血肿2例,严重咽痛、吞咽困难8例,声音嘶哑6例。结论下颈椎骨折脱位围手术期并发症常见,合并呼吸功能障碍较多,应重视围手术期并发症治疗管理,保证围手术期安全,提高临床疗效。更多还原

【Abstract】 Objective To discuss the complications and their therapeutic strategy of lower cervical fractures and dislocations during perioperative period. Methods To summarize clinical datas of 45 cases of lower cervical fractures and dislocations combined with acute spinal cord injuries, and to record and study the complications during perioperative period and their treatment. Results Two patients died of respiratory failure,and the others survived from the perioperative period. During the perioperative period respiratory functional disorder occurred in 15 patients all of whom accepted tracheotomy. Breathing apparatus was used in 4 cases, and pneumonitis occurred in 4 patients. Postoperative hematom occurred in 2 patients, severe pharyngalgia and dysphagia in 8 and hoarseness in 6. Conclusion The complications of lower cervical fractures and dislocations are common,with respiratory functional disorder associated more often during perioperative period. To assure the safety and improve the clinical outcomes,we should think highly of the treatment management of complications during perioperative period.更多还原
