【摘要】 目的分析糖尿病对颈椎椎板成形术手术结果的影响。方法分析接受了颈椎椎板成形术的20例糖尿病患者和20例非糖尿病患者的配对资料。比较两组的术后JOA评分和并发症。评价糖尿病组JOA评分恢复率与标志糖尿病严重程度指标的关系。结果两组JOA总分无统计学差异。但是,糖尿病组术后下肢感觉功能较对照组差。糖尿病组恢复率与术前HbA1含量呈负相关。术后糖尿病组3例患者出现浅层刀口并发症,而对照组未出现刀口并发症。结论①虽然颈椎椎板成形术后糖尿病患者和非糖尿病患者总体恢复旅无显著性差异,但前者术后下肢感觉功能评分显著低于对照组。②恢复率与术前HbA1含量负相关性表明,若想取得良好的手术结果,术前应控制糖尿病至少2到3个月。③糖尿病患者容易出现刀口并发症。更多还原

【Abstract】 Objective To investigate the effects of diabetes mellitus on the s urgical outcome after cervical laminoplasty. Methods 20 patients with diabetes mellitus who underwent cervical laminoplasty and 20 nondiabetic patients who und erwent the same surgical procedure were included. The postoperative score and co mplications were compared between tow groups,the correlation between the recover y rate of JOA score and the factors indicating the severity of diabetes was asse ssed. Results There was no statistical difference between the total JOA score o f the two groups. However,the group with diabetes mellitus showed a poor recover y of sensory function of the lower extremities. A negative correlation was obser ved between the recovery rate and the preoperative HbA1 level in the group with diabetes. The wound complication in group with diabetes mellitus was more than t hat in control group. Conclusion ①Although the cervical myelopathy patients wi th diabetes experienced benefits from cervical laminoplasty similar to nondiabet ic patients,but in sensory function of the lower extremities,the score in group with DM was significantly lower than in the control. ②The negative correlation between the recovery rate and the preoperative HbA1 level suggest that diabetes control of more than 2 to 3 months before surgery is recommended for a favorable surgical outcome. ③the patients with diabetes mellitus were more likely to hav e wound complication.更多还原
