奎硫平与氟哌啶醇治疗精神分裂症兴奋激越症状的对照研究

【摘要】 目的比较奎硫平与典型抗精神病药氟哌啶醇治疗精神分裂症病人伴有兴奋激越的疗效与不良反应。方法符合CCMD-3诊断标准的精神分裂症住院患者,采用随机对照、开放性研究治疗21天。以阳性与阴性症状量表(PANSS)评估疗效,以副反应量表(TESS)评估不良反应。统计学检验采用t检验和非参数检验。结果奎硫平组40例,氟哌啶醇组30例。1.奎硫平与氟哌啶醇的总体疗效(t=1.815)、兴奋症状的控制(t=0.478)效果相当(P>0.05)。2.不良反应方面:氟哌啶醇引起的椎体外系不良反应如肌强直、静坐不能较奎硫平高。结论奎硫平合并氯硝西泮对精神分裂症兴奋症状的疗效与典型抗精神病药物氟哌啶醇相当,不良反应较小。

【Abstract】 Objective: To compare the efficacy and tolerability of oral quetiapine with those haloperidol decanoate in the treatment of schizophrenia with excitement and agitation.Methods: Patients with CCMD-3-diagnosed schizophrenia requiring antipsychotic treatment were randomly assigned to open-label oral quetiapine or haloperidol decanoate for 21days.The Positive and Negative Syndrome Scale was used to assess efficacy;the Treatment Emergent Syndrome Scale was used to assess safety and tolerability.For statistical analyses,t-test and nonparametric tests were used.Results:70 patients were enrolled for the quetiapine(N = 40) and haloperidol decanoate(N = 30) groups.1.T-test analysis showed no between-group differences in the overall efficiency and in controlling excitement and agitation(P>0.05).2.The incidence of extrapyramidal symptoms was low in both groups;patients receiving quetiapine showed significantly greater improvement in rigidity and akathisia.Conclusion:Oral quetiapine was as efficacious as haloperidol in controlling excitement and agitation over 21 days in patients with schizophrenia,with fewer extrapyramidal symptoms,especially rigidity and akathisia.
