子宫动脉化疗栓塞治疗中晚期子宫内膜癌

【摘要】 目的 观察子宫动脉化疗栓塞对中晚期子宫内膜癌的治疗效果。方法 对32 例治疗前中晚期子宫内膜癌患者随机分成二组:A组17例,行双侧子宫动脉超选择化疗栓塞;B组15例,行静脉法全身化疗(对照组)。全部病例完成 2个疗程(每个疗程相隔4~5周)后3~4周复查,根据病灶变化和宫旁情况选择手术或放疗,并对两组病例进行近期疗效和手术切除率比较。结果 子宫动脉化疗栓塞与静脉化疗临床有效率分别为 70. 6%和 26. 7%;手术切除率分别为64.7%和26.7%;两者比较均有明显差异(P<0.05)。结论 子宫动脉化疗栓塞能使中晚期子宫内膜癌癌灶缩小,降低分期,控制淋巴血管内的转移,利于手术切除,减少术中、术后的远处转移。

【Abstract】 Objective To investigate the clinical efficiency of transcatheter uterine arterial chemoembolization for medium-term and terminal endometrial cancer. Methods Thirty-two cases of medium-term and terminal endometrial cancer were randomly divided into two groups. Patients in group A ( n =17 ) were treated by transcatheter uterine arterial chemoembolization, in group B ( n =15) were treated by intravenous chemotherapy for 2 periods with the interval of 4-5 weeks. Then the operation or radio-therapy was executed for all cases according to the changes of the tumor after 3-4 weeks. The clinical efficiency of all cases in the near future and the operative quotiety were compared and assessed. Results The clinical efficiency of group A and B were 70.6% and 26.7%, and the operative quotiety of the two groups were 64.7% and 26.7%, respectively ( P <0.05). Conclusion Transcatheter uterine arterial chemoembolization for medium-term and terminal endometrial cancer can reduce the cancerous focus, debase the cancerous period, control the intravascular and intralymphangial metastasis.
