脊椎压缩性骨折的MRI鉴别诊断

【摘要】 目的:探讨磁共振成像(MRI)在脊椎压缩性骨折临床鉴别诊断中的价值。方法:回顾性分析46例74椎体压缩性骨折患者的临床和MRI影像资料。MR扫描使用GE1.5T机器,检查序列包括平扫T1WI、T2WI、脂肪抑制(STIR),其中12例行Gd-DTPA增强扫描。结果:外伤性脊椎压缩骨折18例,其中新近外伤组8例,陈旧外伤组10例;骨质疏松性脊椎压缩骨折16例;转移性肿瘤致脊椎压缩骨折12例;每组各具有较为特征性的MRI影像表现。结论:MRI诊断脊椎压缩性骨折敏感而准确,可鉴别脊椎骨折是新鲜性的抑或陈旧性的、外伤性的抑或病理性的、良性的抑或恶性的,并有助于指导治疗方案的选择。

【Abstract】 Objective:To assess the clinical differential diagnosis value of magnetic resonance imaging (MRI) in the vertebral compression fracture.Methods:The clinical and MR imaging data of 46 patients with a total of 74 vertebral compression fractures were reviewed respectively.A 1.5-T MR scanner was used.The examination protocol included non-enhanced T1-and T2-weighted fast-spin-echo (FSE), short inversion time inversion-recovery (STIR) sequences and Gd-DTPA enhanced imaging in 12 patients.Results:There were 18 traumatic fractures,including eight patients with recent trauma and ten patients with dated trauma,16 osteoporotic fractures and 12 pathologic compression fractures due to metastatic tumors.Vertebral compression fractures attributed to different causes had distinct MR imaging characteristics. Conclusion:MR imaging is a technique of high sensitivity and accuracy for diagnosing vertebral compression fracture,and it may distinguish recent trauma from dated one,traumatic fracture from pathologic one and benign fracture from malignant one.It plays an important role in clinical differential diagnosis.Moreover,MR imaging may help to direct the option of treatment protocols.
