小剂量布比卡因应用于腰硬联合麻醉下剖宫产手术
The application of The Low Dose Bupivacaine in Combined Spinal and Epidural Anesthesia to Cesarean Section.
【摘要】 目的:观察腰硬联合麻醉(CSEA)小剂量布比卡因(7.5mg)给药在剖宫产手术中的麻醉效果。方法:100例剖腹产患者随机分成腰硬联合(A)组和硬膜外(B)组,每组50例,A组经蛛网膜下腔注入7.5g/L布比卡因1mL,根据麻醉效果,硬膜外加16g/L利多卡因5~10mL;硬膜外组给予16g/L利多卡因10~15mL。观察并记录阻滞平面,麻醉效果,监测术前及用药后5、10、30min的BP、HR、ECG和SPO2和麻醉不良反应。结果:A组麻醉感觉阻滞起效时间、阻滞平面固定时间、手术开始时间短于B组(P<0.01),A组血压和心率先于B组下降,组间比较差异有极显著性(P<0.01);A组麻醉肌松效果好;2组麻醉并发症差异无显著性(P>0.05)。结论:腰硬联合麻醉用小剂量布比卡因在剖宫产手术中安全有效,且并发症不增加。
【Abstract】 Objective:To observe the efficacy and safety of 7.5g/L bupivacaine 1ml in combined spinal and epidural anesthesia (CSEA) for cesarean section.Methods:100 patients undergoing cesarean section were randomly devided into combined spinal and epidural with low dose bupivacaine (group A,n=50) and epidural anesthesia (group B,n=50).group A :7.5g/L bupivacaine 1mL for lumbar anesthesia,during operation when lumbar anesthesia was inadequate,16g/L lidocaine 5~10 mL was supplemented via epidural catheter.group B:16g/L lidocaine 10~15 mL for epidural anesthesia.Sensory block time,beginning time of operation,efficacy of analgesia and muscle relaxation were assayed.Blood pressure,heart rate and SpO 2 were gained before anesthesia,and 5、10、 30 min after the injection of the drug.And anesthesia complications were assayed. Results:The beginning time of effective anesthesia,the block time of the height and the beginning time of operation in group A were shorter than group B(P<0.01); MAP and HR in group A decreased faster than group B(P<0.01),Analgesia and muscle relaxation in group A were more satisfactory than group B. there was no significantly difference in anesthesia complications between the two groups(P>0.05).Conclusion:Bupivacaine 7.5mg is effective and safe in CSEA for cesarean section with no increase in the complications.
