肺源性心脏病患者缺氧的量化判定与动脉血气分析的相关性研究

【摘要】 目的寻找一种判定肺源性心脏病(简称肺心病)患者缺氧程度的方法,为无条件进行血气分析的医务人员提供诊治、护理参考。方法(1)以急性生理功能和慢性健康状况评分系统II为框架,自行设计肺心病患者缺氧的量化评分表,对67例肺心病患者缺氧状况进行评分;(2)将评分结果与动脉血气分析进行对比分析,找出其相关性。结果该量表的缺氧评分与pH、PaO2、SaO2有负相关性,与PaCO2有高度正相关性,P均<0.01。结论缺氧量表为临床和基层医疗单位提供了一种简便有效的诊治、护理参考依据,基本反映了肺心病患者缺氧的临床症状和缺氧程度。

【Abstract】 Objective To find a method of symptomatic evaluation of hypoxia in patients with pulmonary heart disease so as to provide care givers with a diagnostic option when there is no condition for blood gas analysis.Methods (1) Based on the acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ) system,the authors designed a quantitative evaluation chart for hypoxia in patients with pulmonary heart disease whereby the hypoxic condition was scored in 67 patients with pulmonary heart disease;(2)The results obtained were compared with those of arterial blood gas analysis to see whether there was any correlation between them.Results The score was negatively correlated with pH,PaO2 and SaO2(P<0.01),and positively correlated with PaCO2 (P<0.01).Conclusion The quantitative chart for evaluating hypoxia provides a simple and effective diagnostic option for root-level medical units where there is no condition for blood gas analysis in that it basically reflects the clinical manifestations and degree of hypoxia in patients with pulmonary heart disease.
