【摘要】 目的讨论体外受精—胚胎移植(IVF-ET)后发生重复输卵管妊娠的病因及处理。方法回顾性分析我院收治的1例IVF-ET后输卵管重复妊娠患者的临床资料,并复习文献。结果患者32岁,有右侧输卵管积水和左侧输卵管妊娠病史,在本中心行IVF-ET后发生左侧输卵管双卵双胎妊娠,予手术切除左输卵管。2年后,行解冻胚胎移植获宫内妊娠和重复输卵管妊娠,予期待治疗输卵管妊娠,宫内妊娠得到继续生长发育,终获一活女婴。结论输卵管病变及既往有输卵管妊娠史是导致胚胎移植后发生重复输卵管妊娠的主要病因。IVF-ET后发生输卵管妊娠时,应针对输卵管病变的严重程度选择有效治疗,以减少输卵管妊娠的反复发生。多部位妊娠时,及时发现异位妊娠,正确处理,宫内妊娠可获良好结局。更多还原

【Abstract】 Objective To investigate the cause, treatment and prevention of repeated ectopic pregnancy after in vitro fertilization and embryo transfer(IVF-ET). Methods The clinical data of one patient with repeated fallopian pregnancy after IVF-ET in our hospital was analyzed retrospectively, and the related literature was reviewed. Results The patient haddisease history of the right hydrosalpinx and the left fallopian pregnancy. After IVF-ET inour center the patient suffered from fallopian twin pregnancy and was treated by excising theleft fallopian. Two years later by transfering cyropreserve-thraw embryo, the patient hadboth intra- and extra-uterine pregnancy. After extrauterine pregnancy was treated expected,intrauterine pregnancy continued to develop and an alive female baby was born. Conclusion Themajor cause of repeated ectopic pregnancy is the pathological changes of the fallopians and thehistory of extrauterine pregnancy. In ectopic pregnancy of IVF-ET what can prevent repeatedectopic pregnancy is the effective treatment of pathological changes of the fallopians. If thereis ectopic pregnancy in multiple sites and extrauterine pregnancy is treated effectively andpromptly, intrauterine pregnancy will has a good outcome.更多还原
