【摘要】 目的:用连续性血液滤过技术(CRRT)防治烧伤脓毒症,减轻炎性介质对机体的进一步损害,提高严重烧伤病人治愈率。方法:4例特重烧伤病人休克期后出现烧伤脓毒症,采用德国贝朗(BRAUN)CRRT5.3机,AV600S血滤器,进行连续性血液滤过治疗,每天12～14h,连续5～7d。治疗前后观察生命体征,检测电解质、血尿素氮、血气、肿瘤坏死因子(TNFα)等指标,根据病情及检测结果调整滤过液的配制。结果:治疗前后患者各生命体征及各监测指标均有不同程度的改善,大多数指标治疗前后变化有统计学意义(P<0.05)。结论:CRRT技术应用于大面积烧伤病人,可清除炎症介质和毒素,维持内环境稳定,补充营养,是防治烧伤脓毒症的一种有效治疗手段。更多还原

【Abstract】 Objective: Applying CRRT to prevent burn sepsis, reduce the damage of the inflammation medium to the body and improve the cure rate of the serious burn patients.Methods: Four cases of serious burn patients had received CRRT treatment for five to seven days and twelve to fourteen hours a day, who were post shock or had burn sepsis. We observed the patients’ life symptoms and checked the index of water and electrolyte, blood uria nitrogen, sap and TNF-α before and after CRRT. We adjusted the dispensing of the filtered liquid according to the index.Results: The life symptoms and the detected index were improved with different degree before and after the treatment. Most of the index had statistics significance (P<0.05).Conclusion: The technique of CRRT applied in large area burn patients may delete inflammation medium and poison, maintain the stability of the inside circulation and fill up nutrition, which is an effective treatment method to prevent burn sepsis.更多还原
