【摘要】 目的　探讨慢性阻塞性肺病 (COPD)以及COPD合并阻塞性睡眠呼吸暂停低通气综合征 (OSAHS) (以下称重叠综合征 )患者的肺功能与睡眠特点。方法　观察对象 4 6例 ,其中COPD组 2 9例 ,重叠综合征组 17例 ,4 6例全部行多导睡眠图(PSG)监测及肺功能测定 ,并比较 2组肺功能指标、呼吸暂停低通气指数 (AHI)、呼吸暂停时间及夜间睡眠最低血氧饱和度(LSaO2 )。结果　重叠综合征组的 1s用力呼气容积 (FEV1)、用力肺活量 (FVC)、最大通气量 (MVV)、最大呼气中段流量 (MMEF)均显著低于COPD组 (P <0 0 1和P <0 0 5 ) ;重叠综合征组的AHI、呼吸暂停时间显著高于COPD组 ,LSaO2 显著低于COPD组。结论　COPD合并OSAHS呼吸紊乱严重 ,加重低氧血症 ,明显影响肺功能。更多还原

【Abstract】 Objective To study the characteristics of lung function and sleep in patients with chronic obstructive pulmonary disease (COPD) and COPD complicated obstructive sleep apnea hypopnea syndrome (OSAHS).(COPD with OSAHS is called overlap syndrome. Methods Forty-six subjects were detected by polysomnography and lung function,then they were divided into 2 groups according to apnea hypopnea index (AHI)and lung function.29 patients were included in COPD group,17 patients were in overlap syndrome group.Lung function included forced expiratory volume in one second (FEV1),forced vital capacity (FVC),maximal voluntary ventilation(MVV) and maximal midexpiratory flow curve (MMEF).Change of lung function、AHI、sleep apnea time and the lowest oxygen saturation (LSaO 2 ) at night were compared in two groups.Results FEV1、FVC、MVV、MMEF in overlap syndrome group was significantly lower than those in COPD group(P<0.01,P<0.05).AHI and sleep apnea time in overlap syndrome group were significantly higher than those in COPD group(P<0.01,P<0.05).LSaO 2 in overlap syndrome group were significantly lower than that in COPD group(P<0.01 ).Conclusions COPD with OSAHS could aggravate sleep-induced respiratory abnormalities and hypoxemia,and obviously affect lung function.更多还原
