在急性脑梗死溶栓治疗中F1+2、TAT和D-二聚体的应用研究
Research on F1+2,TAT and D-D in treating acute cerebral infarction with thrombolytic therapy
【摘要】 目的:探讨急性脑梗死溶栓治疗前后血中凝血酶原片段1+2(F1+2)、凝血酶-抗凝血酶-复合物(TAT)和D-二聚体(D-D)的动态变化及临床意义。方法:对42例急性脑梗死患者(均于6h内接受溶栓治疗)溶栓前及溶栓后2,4,8,24,48h血标本采用酶联免疫吸附双抗体夹心法(ELISA)测定F1+2、TAT、D-D水平。其中有效组28例,无效组14例,并与正常对照组(n=42)进行比较。结果:治疗组治疗前后与正常对照组相比F1+2、TAT、D-D差异均有显著意义(P<0.05)。溶栓后有效组2hF1+2、D-D水平达到峰值,2h后D-D水平迅速下降,但48h仍然高于治疗前水平3倍以上。无效组F1+2、TAT持续缓慢上升,D-D4~8h低于有效组,但3项指标24h均达到峰值。结论:急性脑梗死溶栓治疗前后F1+2、TAT、D-D水平变化对判断疗效有一定指导意义。
【Abstract】 Objective:To explore the changes and clinical significance of F1+2,TAT and D-D during treating acute cerebral infarction with thrombolytic therapy.Methods:42 cases of acute cerebral infarction (treatment group) and 42 normal control subjects (control group) were studied.Before thrombolytic therapy and at 2,4,8,24,48h after thrombolytic therapy,F1+2,TAT and D-D levels were measured by ELISA method and the data were analyzed.Results:There were the significant differences of F1+2,TAT and D-D between the two groups(P<0.05).After thrombolytic therapy,the efficient cases showed the peak levels of F1+2 and D-D and then,D-D level decreased rapidly at 2h,but still keep as high as three times of that before treatment.Non-efficient cases showed the slow increase of F1+2 and TAT,and the three indeies reached the peak at 24h after treatment.Conclusion:Changes of F1+2,TAT and D-D levels before and after thrombolytic therapy may reflect the treatment efficacy and it can be used as the guidance indicator to treat acute cerebral infarction.
