高危型人乳头瘤病毒检测及其临床意义

【摘要】 目的 探讨人乳头瘤病毒 (HPV)与宫颈癌的相关性及广东地区高危型HPV感染的调查。方法 第二代杂交捕获法 (HCⅡ )检测 1994例妇女宫颈刷出物中的高危型HPV。结果 宫颈癌患者HPV感染率显著高于健康体检者及宫颈炎患者 ,差异具有显著性 (P <0 0 1)。 1994例妇女高危型HPV感染率为 2 7 7% ,广州、深圳、东莞、新会 4城市分别为 2 4 9% ,34 2 % ,31 7%和 17 1% ,新会与其他 3个城市比较差异有显著性 (P <0 0 1)。结论 高危型HPV是诱发宫颈癌的重要病因学因素 ,控制HPV感染可降低宫颈癌的发病率。

【Abstract】 Objective To evaluate the etiological role of human papillomavirus in cervical carcinoma and to investigate high-risk HPV infection rate of Guangdong province. Methods Hybrid capture Ⅱ test was used to detect 13 high-risk HPV types from 1994 cervical scrapes. Results 12(92.5%) of the 13 women with cervical carcinoma were infected with high-risk HPV verus 22(20.6%) of 107 women attending routine cervical carcinoma screening(P<0.01). HPV were found in 112(31.3%) of 358 cervical erosion. The infection rate of high-risk HPV was significantly different between cervical erosion group and cervical carcinoma group(P<0.01). Among 1994 women, 540(27.7%) were found high-risk HPV positive. There was a peak infection rate(54.0%) of high-risk HPV among ≤20 years age of group. 41~50 years of age group has the lowest infection rate(20.6%). The corresponding infection rate of high-risk HPV in Guangzhou, Shenzhen, Dongguan and Xinhui was 24.9%, 34.2%, 31.7% and 17.1%, respectively. There was significant difference between Xinhui and Guangzhou(P<0.01), Xinhui and Shenzhen(P<0.01), Xinhui and Dongguan(P<0.01). Conclusion High-risk HPV is the major cause of cervical carcinomas and controlling high-risk HPV infection can decrease the incidence of cervical carcinoma.
