冠心病心力衰竭患者左心室非同步运动的组织速度图特征

【摘要】 目的探讨定量组织速度成像技术(QTVI)评价冠心病心力衰竭患者左心室非同步运动的价值。方法选择正常人及冠心病心衰(CHF)患者各36例,利用QTVI技术,获取标准心尖部左室长轴、两腔、四腔观的左心室6个壁基底段、中间段和心尖段共18个位点的多普勒速度曲线,测量各位点QRS波起始至心肌收缩峰值速度和舒张早期峰值速度的时间(Ts和Te),并计算同一节段6个壁间的最大差值(△Ts1、△Te1)、同一壁内3个节段间Ts和Te的最大差值(△Ts2、△Te2)及左室18个位点间Ts和Te的最大差值(△Ts3、△Te3)。同时计算二尖瓣环6个位点收缩期峰值速度和舒张早期峰值速度的均值(mVs、mVe)及射血分数(LVEF)。结果CHF组△Ts1、△Te1、△Ts2、△Te2、△Ts3、△Te3较正常组明显延长(P<0.05);CHF组△Ts3、△Te3与mVs、LVEF呈负相关(r=-0.721,r=-0.748,P<0.01);△Te3与mVe呈负相关(r=-0.628,P<0.01)。结论冠心病心衰患者左心室同一节段及同一壁内均存在非同步运动,且与左心功能关系密切。

【Abstract】 Objective To evaluate the asynchrony of the different segments of left ventricle for the patients with congestive heart failure of coronary heart disease by quantitative tissue velocity imaging(QTVI).Methods Thirty six normal controls and 36 patients with CHF were studied by QTVI.The regional velocity profiles of 18 segments of LV apical long-axis view,4 chamber and 2 chamber view were obtained.The time from onset of QRS complexes to peak velocity during systole and early diastole(Ts and Te)were measured.The maximal difference in Ts and Te within the same wall(△Ts1 and △Te1 ),the same segment(△Ts2 and △Te2)and all 18 segments(△Ts3 and △Te3)of LV were calculated.The mean between mitral annulus systolic peak velocities and early diastolic peak velocities of six walls(mVs and mVe),and ejection fraction of LV(LVEF)were calculated.Results △Ts1,△Te1,△Ts2,△Te2,△Ts3 and △Te3 in CHF group were significantly prolonged when compared with control group(all P<0.05).△Ts3 and Te3 were correlated negatively with mVs and LVEF respectively(r=-0.721,P<0.01;r=-0.748,P<0.01).△Te3 during diastole was correlated negatively with mVe(r=-0.628,P<0.01).Conclusions Asynchrony exists within the same wall and the same segments of LV,having close relation-ship with LV function of the patients with CHF of Coronary artery disease.
