【摘要】 目的探讨大龄儿童发育性髋关节脱位的手术治疗方法。方法对于大龄儿童发育性髋关节脱位多部位截骨矫形手术,髋骨截骨切骨线前后呈“L”形,外高内低,耻骨联合正中偏外侧1cm耻骨截骨,髋臼骨块得到骨性游离,向前、外、下方旋转适当角度,取三角形髂骨内外板骨块或股骨短缩旋转截除的骨块嵌塞于前方三角形裂隙,用2枚克氏针固定,耻骨截骨处用1枚克氏针固定,紧缩缝合关节囊,粗隆下股骨旋转截骨纠正前倾角和股骨头上移。术后采用改良的贝氏石膏固定。结果治疗大龄儿童发育性髋关节脱位20例23髋,无严重近期并发症发生,随访0.5～6年,无股骨头坏死发生及再脱位,术后髋臼指数平均27°,平均纠正前倾角65°。双下肢长度差异降至平均1.2cm。优13髋,良7髋,可3髋,优良率86.9%。结论多部位联合截骨术一次性纠正多部位主要病理改变,疗效肯定,并发症少,适合于大龄儿童发育性髋脱位的治疗。更多还原

【Abstract】 Objective To search a more effective proceduse for the treatment of developing dislocation of the hip(DDP)in elder children.Methods Twenty cases (23 hips)with DDP were operated on by combined multi-section osteotomy including the modified innominnatum iliac bone,pubic bone and the femoral bone.A modified plastic instrument was used after the operation.Results There were no severe complications after operation.Thirteen hips were excellent and 7 hips good in 20 cases(23 hips)after 6 months to 6 years’ fowllow-up.The excellent and good rate was 93.3%.There was no redislocation or sublaxation.Conclusion Combined multi-section ostiotomy with modified plastic instrument after operation is an effective method for the treatment of DDP in elder children with satisfactory results.更多还原
