【摘要】 目的　评价经睫下皮肤切口与鼻内窥镜下经筛窦联合入路行眶爆裂性骨折整复术的应用效果。方法　分析 3 1例采用经睫下皮肤切口与鼻内窥镜下经筛窦联合入路行眶爆裂性骨折整复术病人的临床资料。结果　术后 2周内检查 3 1例复视均好转 ,术后 1 5个月到 8个月检查复视治愈 2 7例。 2 7只眼眼球活动度完全改善 ,4只眼眼球活动度部分改善。 3 1例术后 7～ 10天伤眼眼球突出度增至 10 5～ 14 5mm ,平均 13 3mm ;术后伤眼与健眼眼球突出度差为 + 1 5～ -2 0mm ,平均 +0 9mm。手术后追踪观察 3个月～ 3年 ,无并发症发生。结论　本术式具有安全可靠 ,皮肤瘢痕轻微 ,低创伤 ,视野清晰 ,不损伤视神经及眶下神经 ,操作简便和术后合并症少等特点 ,对矫正眼球内陷及消除复视有确切疗效 ,并符合面部美容要求 ,是目前治疗眶爆裂性骨折的理想术式。更多还原

【Abstract】 Objective To evaluate the therapeutic effects of the double approaches of the ethmoid sinus under nasal endoscope and the subciliary incision on orbital blowout fracture.Methods 31 cases with orbital blowout fracture who received the orbital restitution through double approaches of the ethmoid sinus under nasal endoscope and the subciliary incision were analyzed.Results Diplopia of the 31 cases were all diminished within 2 weeks after operation and disappeared in 27 cases at 1 5～8 months after operation.The mobility of the globes recovered completely in 27 cases and was improved partially in 4 cases.The degree of the ocular proptosis gained 10 5～14 5mm and was 13 3mm averagely at 7～10 days after operation.The difference of proptosis between the healthy eye and the suffering eye remained +1 5～-2 0mm and averagely was +0 9mm postoperatively.They recovered with no complications in a 3 months′ to 3 years′ follow-up.Conclusions The approach is characterized by its safety and reliability,slight scar on the skin,mild wound to the patient,clear field intraoperatively,no harm to the optic nerve and suborbital nerve,simple work during the process,and low possibility of complications postoperatively.It has positive therapeutic effects on enophthalmos and diplopia caused by trauma and matches the needs of facial appearance.Therefore,it is an ideal approaches for the orbital blowout fracture.更多还原
