醛糖还原酶基因多态性与糖尿病视网膜病变的相关性研究

【摘要】 目的 探讨醛糖还原酶 (aldosereductaseAR)基因BamHⅠ酶切位点多态性和中国人Ⅱ型糖尿病伴视网膜病变之间的相关性。方法 用PCR -RFLP的方法 ,检测了 10 0例Ⅱ型糖尿病伴视网膜病变的患者 (5 0例增殖型及 5 0例单纯型视网膜病变 ) ,78例对照的基因多态性。比较各组间等位基因型及等位基因频率的分布。结果 ①全部研究对象中共见有三种基因型 ,BB纯合子等位基因型 ,bb纯合子等位基因型 ,Bb杂合子等位基因型。②单纯型糖尿病视网膜病变 (nonproliferativedi abeticretinopathyNPDR)组B等位基因的基因频率较增殖型糖尿病视网膜病变 (proliferativediabeticretinopathyPDR)组和对照组均有增高 ,差别存在显著性 (χp2 =11 60 ,P <0 0 5 ;χp2 =4 95 ,P <0 0 5 )。结论 B等位基因的表达可能有高的AR活性 ,促进早期糖尿病视网膜病变的发生 ,但多元醇通路的激活并不能直接增加增殖性糖尿病视网膜病变发生的危险性 ,有关AR对糖尿病视网膜病变影响还需进行大量、细致的研究工作才能得以阐明。

【Abstract】 Objective To investigate the correlation between aldose reductase BamHⅠ polymorphism and diabetic retinopathy in chinese NIDDM patients.Methods PCR and restriction fragment length polymorphism analysis of aldose reductase gene was performed in 100 NIDDM patients with retinopathy(including 50 proliferative diabetic retinopathy and 50 nonproliferative diabetic retinopathy)and 78 non-diabetes control subjects.Results Three genetypes were found(BB,Bb,and Bb),The frequecy of allele B was significantly higher in NIDDM patients with nonproliferative diabetic retinopathy than that in NIDDM patients with proliferative diabetic retinopathy and controls.(χp 2=11.60, P <0.05;χp 2=4.95, P <0.05).Conclusions Expression of B allele may increase the activity of aldose reductase,lead to a early event in the development of diabetic retinopathy,but the activation of polyol pathway may not enhance the susceptivity of proliferative diabetic retinopathy,and mass,definite study should be performed to clarify the mechanism between AR and diabetic retinopathy.
