腓肠神经营养血管逆行岛状皮瓣的临床应用
Clinical Application of Distally Based Neurovascular Island Flap Pedicled with the Sural Superficial Nerve
【摘要】 目的探讨改进的腓肠神经营养血管的逆行岛状皮瓣修复足跟部、踝部及足背软组织缺损的方法。方法对12例足跟部、踝部及足背软组织缺损,采用选择性结扎小隐静脉及蒂部保留梨形皮桥,带腓肠神经营养血管蒂岛状皮瓣逆行转位修复。皮瓣面积最大为19 cm×12 cm,最小为8 cm×5 cm。结果12例皮瓣全部成活,经10~12个月随访,效果满意。结论改进的腓肠神经营养血管逆行岛状皮瓣血供充分,有利于皮瓣静脉回流,提高皮瓣成活率,为临床治疗较困难的足跟部、踝部及足背软组织的缺损,提供了一个较为可靠的方法。
【Abstract】 Objective To report the modified distally based neurovascular fascial flap pedicled with the sural superficial nerve for repairing defect at the heel,ankle and foot dorsurm.Methods 12 patients with soft tissue defects at the foot dorsum,heel and ankle were treated with the distally based reverse neurovascular fascial flaps pedicled with the sural superficial nerve in which the small saphenous vein were selectively ligated and skin bridge was kept in the shape of the pear on the site of the pedicel.The size of the flaps ranged from 19 cm×12 cm to 8 cm×5 cm.Results All flaps survived with satisfactory results.The follow-up period was 10~12 months.Conclusion This flap has sufficient blood supply and vein drainage,assuring high survival rate.It is a reliable method for the treatment of refractory defect at the foot dorsum,heel,ankle.
