胎儿窘迫时羊水中部分生化成分变化的研究
Changes in biochemical ingrdients of amnionic fluid following fetal distress
【摘要】 目的 探讨羊水中部分生化成分的变化对诊断胎儿窘迫的价值。方法 随机选择 5 0例正常产妇作正常组 ,5 0例胎儿窘迫者作为胎儿窘迫组 ,取各组羊水进行生化测定分析 ;每例新生儿出生时行Apgar 1min及 5min评分。结果 胎儿窘迫组中 ,羊水中乳酸浓度 (7.2 5± 1.2 8mmol/L)、乳酸脱氢酶浓度 (32 4 .98± 10 7.6mmol/L)、尿素氮浓度 (7.0 9± 1.16mmol/L)、肌酐浓度 (0 .185± 0 .0 2 8mmol/L)较正常组乳酸浓度 (5 .95± 0 .6 7mmol/L)、乳酸脱氢酶浓度 (114 .4 1± 5 6 .2mmol/L)、尿素氮浓度 (4.2 3± 0 .98mmol/L)及肌酐浓度 (0 .13± 0 .0 3mmol/L)显著升高 (P<0 .0 0 0 5 ,P <0 .0 5 ,P<0 .0 0 5 ,P<0 .0 0 0 5 )。窘迫组的新生儿窒息率 (12 % )较正常组 (2 % )显著升高 (P <0 .0 2 5 )。结论 胎儿窘迫时 ,羊水中乳酸、乳酸脱氢酶、尿素氮、肌酐浓度较正常显著升高 ,此种变化可反映胎儿体内的代谢状况 ,为胎儿窘迫的诊断提供一些客观依据。
【Abstract】 Objective: To quantify the biochemical ingredients of amnionic fluid following fetal distress. Method: 50 cases of natural puerperas were selected in a randomized manner as natural group;50 cases of pueperas with fetal distress as fetal distress group;In every case, amnnionic fluid was drawn and measured its biochemical ingredients. All babies had been done Apgar-score on delivery of one and five minute. Result: Compared natural group and fetal distress group, lactic acid, lactic dehydrogenase, urea nigtrogen and creatinine consentrations were significantly increased( P<0.0005, P<0.05,P<0.005,P <0.0005). The fetal distress group had high incidence of neanatal asphyxia as compared with the natural group ( P <0.025). Conclusion: The biochemical ingredients of amnionic fluid had changed as fetal distress. So quantify the biochemcal ingredients can provide the proof of diagnosing fetal distress.
