多普勒组织成像技术诊断胎儿心律失常

【摘要】 目的 探讨应用多普勒组织成像技术 (DTI)诊断胎儿心律失常的可行性。方法 对产前检查发现有心律失常的 5 7例胎儿应用DTI的二维和脉冲方式观察四腔心切面的房室瓣环运动 ,分析其图像特征 ,并与传统的脉冲多普勒血流频谱 (PW )进行比较。结果  5 7例胎儿DTI全部当次获得满意图像 ,并能根据其图像特征进行分类诊断 ;应用PW有 3例胎龄小于 18周的胎儿因无法显示五腔心切面不能进行分类诊断。同一胎儿DTI与PW对比诊断结果一致 ,同类型胎儿心律失常DTI脉冲方式与PW的图像特征一致。结论 DTI可以诊断胎儿心律失常。

【Abstract】 Purpose To investigate the possibility of Doppler tissue imaging(DTI) diagnosis of fetal arrhythmias. Methods DTI examined 57 fetuses with 14-40 weeks gestational age that suffer from arrhythmia, observed and analyzed motion amplitudes and peak distance of the mitral annulus and the tricuspid annulus, and compared the characteristic of Pulsed Doppler tissue imaging(PDTI) and traditional Pulsed-wave Doppler(PW). Results 57 fetuses were obtained satisfied imaging and were diagnosed all in one times with DTI. PW didn’t obtained satisfied imaging of 8 fetuses in one times because couldn’t get five-chamber views. 5 fetuses could and 3 fetuses with 14-16 weeks gestational age couldn’t obtain satisfied imaging when rechecked after 24 hours. The diagnosis and characteristic of PDTI is consistent with that of PW. Conclusions DTI is useful in diagnosis of fetal arrhythmias.
