多层CTA三维重建技术评价脑动脉瘤的临床应用:与DSA对比研究
Three-dimensional multi-slice CT angiography in evaluating cerebral aneurysm:comparison with digital subtraction angiography
【摘要】 目的 探讨多层CTA三维重建 (3D MSCTA)与脑血管造影数字减影术 (IADSA)诊断脑动脉瘤的临床价值。方法 对 3 7例自发性蛛网膜下腔出血患者行 3D MSCTA及IADSA检查。以手术或随访为标准评价两种检查结果及图像质量。结果 发现脑动脉瘤 2 9例 3 4处。两种检查的敏感度、阳性预测值、阴性预测值间有极显著性差异 (P <0 .0 1) ,特异度、准确度间及VR/MIP、IADSA敏感度和阴性预测值间有显著性统计学差异 (P <0 .0 5 )。图像质量评估数值最高为VR ,组间有显著性差异 (P <0 .0 1)。结论 对于脑动脉瘤的诊断 ,3D MSCTA比DSA更敏感、更准确。
【Abstract】 Objective To evaluate the clinical value of three-dimensional multislice CTA and digital subtraction angiography for diagnosis of cerebral aneurysms. Methods Thirty-seven patients who had subarachnoid hemorrhage underwent 3D-MSCTA and IADSA examinations. All results of 3D-MSCTA and IADSA examinations and image quality of three reconstruction techniques of 3D-MSCTA and IADSA were assessed. Results Twenty-nine patients had 34 cerebral aneurysms. There was significant difference in sensitivity,positive predictive value and negative predictive value between 3D-MSCTA and IADSA ( P <0.01). Besides, there was statistical difference between specificity and accuracy ( P <0.05). Statistical difference was also found in sensitivity and negative predictive value between VR/MIP and IADSA ( P <0.5). For the score of image quality, VR was the highest, markedly significant differences were demonstrated between the two techniques ( P <0.01). Conclusion 3D-MSCTA was proved to be more sensitive and accurate than IADSA in evaluating cerebral aneurysms.
