超声技术评价卡维地洛对糖尿病血管内皮功能的作用

【摘要】 目的 评价超声观察卡维地洛对糖尿病患者肱动脉内皮依赖性舒张功能的作用。方法 应用高频超声对4 5例糖尿病患者卡维地洛治疗前后和 30例正常对照者的肱动脉内皮依赖性舒张和非内皮依赖性舒张功能进行检测。结果 治疗前糖尿病患者肱动脉充血反应和硝酸甘油引起的舒张较正常对照组明显减弱 ,反应性充血分别为 (3.86±2 .33) %和 (13.2 5± 4 .16 ) % (P <0 .0 1) ;硝酸甘油分别为 (8.4 4± 3.3) %和 (2 0 .4 6± 4 .78) % (P <0 .0 1)。糖尿病患者服用卡维地洛 6个月后 ,反应性充血介导的舒张较糖尿病非卡维地洛组明显增加 ,分别为 (6 .92± 2 .1) %和 (3.79± 2 .4 1) % (P <0 .0 5 ) ,两组对硝酸甘油介导的肱动脉扩张差异无统计学意义。结论 卡维地洛能明显改善糖尿病患者内皮依赖性扩张功能。

【Abstract】 Objective To evaluate the therapeutic effects of carvedilol on endothelial function of brachial artery in patients with diabetes mellitus (MD) by ultrasonography.Methods Endothelium function of the brachial artery in 45 patients with MD and 30 normal controls was studied by high-resolution ultrasound. There were twenty-four cases with MD taking carvedilol for six months and 21 cases without carvedilol. Results Flow-mediated dilatation in brachial arteries in the patients with MD decreased significantly comparing with the normal controls (3.86±2.33)% vs (13.25±4.16)%,P<0.01. The vasolilation response to nitroglycerin in the patients with MD also weaker than normal controls (8.44±3.3)% vs (20.46±6.78)%,P<0.01. Flow-mediated dilatation was increased in MD was after taking carvedilol for six months comparing with no carvedilol (6.92±2.1)% vs (3.79±2.41)%,P<0.05. In terms of nitroglycerin-induced dilatation, there was no difference between patients with and without taking carvedilol. Conclusion These findings suggest that carvedilol can improve the endothelium-dependent dilatation in patients with diabetes mellitus.
