【摘要】 目的 :探讨复杂性肠梗阻的有效治疗方法 ,评价中西医结合疗法的优势。方法 :首先采用积极的中西医结合非手术疗法治疗复杂性肠梗阻 ,包括中药和肠内、肠外营养支持。然后酌情进行根治性小肠排列术或者长期中药治疗。结果 :4 5例中有 12例慢性不全性肠梗阻和 33例急性肠梗阻。急性肠梗阻病例中有 2 4例尚能接受非手术治疗 ,缓解率为 5 8.3% (14 / 2 4 )。共有 16例接受了择期小肠排列术 ,19例接受了急症小肠排列术 ,手术的近期有效率为 94 .3% (33/ 35 ) ,远期有效率为 96 .7% (2 9/ 30 ) ;急症手术病死率为 10 .5 % (2 / 19) ,择期手术无死亡病例 ,并发症也明显减少。另有 10例急性梗阻缓解后长期采取中药治疗。结论 :中西医结合疗法对复杂性肠梗阻的急性病例和慢性病例均有明显疗效。小肠排列术可获得根治性效果 ,但急症手术风险较大 ,应该尽量避免更多还原

【Abstract】 Objective: To investigate the effective therapy and evaluate the superiority of integrated traditional Chinese and western medicine(TCM WM) in treating complicated intestinal obstruction. Methods: First, complicated intestinal obstruction was treated with nonoperative TCM WM, including traditional Chinese medicine, enteral nutrition and total parenteral nutrition. Second, performed radical operation (intestinal plication) or longterm nonoperative TCM WM treatment was used. Results: Of 45 cases, there were 12 cases of chronic complicated intestinal obstruction and 33 cases of acute one.Among 33 cases, 24 still had chance to receive nonoperative therapy and relived rate was 58 3%(14/24). Sixteen cases received selective operation and 19 cases received acute operation. The shortterm effective rate was 94 3%(33/35) and longterm one was 96 7%(29/30). The mortality of acute operation was 10 5%(2/19). However, selective operation had no dead case and much lower complication rate.Another 10 cases had got longterm effect with nonoperative therapy after reliving from acute obstruction. Conclusion: TCM WM therapy is very effective for both acute and chronic cases of complicated intestinal obstruction. Intestinal plication can get radical result,but emergency operation takes risks and should be avoid as you can as possible.更多还原
