多发伤病人外周血髓系细胞触发受体-1的表达与意义

【摘要】 目的探讨多发伤病人外周血中髓系细胞触发受体-1(TREM-1)转录水平的表达规律及其临床意义。方法收集30例临床多发伤病人和7例健康志愿者外周血,提取白细胞,以半定量RT-PCR检测外周血白细胞TREM-1 mRNA水平表达,并分析其临床意义。结果对照组TREM-1 mRNA半定量比值为0.48±0.072,多发伤病人1,2,3和7 d外周血TREM-1 mRNA半定量比值分别为0.89±0.063,0.94±0.070,0.83±0.095,0.66±0.063,差异有显著意义(P<0.05)。伤后第1天和第2天,ISS≥16组TREM-1表达显著高于ISS<16组(P<0.001)。伤后第2天和第7天,脓毒症组表达水平显著高于非脓毒症组(P<0.01 )。结论多发伤病人TREM-1 mRNA表达升高,在创伤早期与ISS评分相关,且在脓毒症的发生中起重要作用。

【Abstract】 Objective: To investigate the expression status of human TREM-1 mRNA, to get complete TREM-1 cDNA., and determine the feasibility of its prokaryotic expression. Methods: Peripheral blood of 30 severe trauma patients and 7 healthy volunteer were collected. TREM-1 mRNA was determined by semi-quantitative reverse transcriptional polymerase chain reaction, and its clinical significance was analysed. Results: The values of TREM-1/β-actin of control group was 0.48±0.072, while those of trauma group in 1 d, 2 d, 3 d, 7 d were 0.89±0.063, 0.94±0.070, 0.83±0.095, 0.66±0.063 respectively (P <0.05). In 1 d and 2 d, TREM-1/β-actin value of patients with ISS ≥16 were significantly higher than those of patients with ISS<16 (P <0.01). In 2 d and 7 d, TREM-1/β-actin values of patients with sepsis was significantly higher than those of patients without sepsis (P <0.01). Conclusion: TREM-1 mRNA expression is up-regulated in patients with multiple trauma, and it is related with higher ISS and onset of sepsis.
