射频消融术后CTnT动态变化的研究(英文)

【摘要】 目的 本研究旨在评价电化学发光法定量检测血清肌钙蛋白T(cTnT)在诊断心肌轻度损伤的价值。方法 对 3 9例接受射频消融术 (RFCA)患者进行射频消融术前、术后 3 0min、4h、12h、2 4h血清cTnT、CK-MB测定及 3 0例健康对照者血清cTnT、CK -MB测定。结果  3 9例患者中 ,2 9例cTnT超过参考值 (P <0 .0 0 5)占 74% ,9例患者CK -MB超过参考值 (P >0 .0 5)占 2 3 % ,cTnT升高的百分数与CK -MB升高的百分数比较有显著性差异 (P <0 .0 5) ;3 9例患者射频消融术前、术后 4个不同时间均有显著性差异 (P <0 .0 5) ,而CK -MB无显著性差异 (P >0 .0 5)。结论 cTnT是反映心肌细胞损伤的灵敏性、特异性均较好的生化指标 ,对心肌微小损伤的诊断和疗效观察均有重要意义。

【Abstract】 Objective: To evaluate the diagnostic value of serum cardiac troponinT(cTnT) quantitatively determined by Electroeherailuminescence Immunoassay in minor myocardial damage. Methods: A total of 39 patients were underwent successful RFCA, serum cTnT level and creatine kinase isoenzyme MB(CK-MB) activity were measured respectively before, 0.5, 4, 12 and 24 hours after the procedure, cTnT and CK-MB were simultaneously measured in 30 healthy control subjects.Results: The elevation of the cTnT level was found in 74%(29/39) of the patients, P <0.005; the elevation of CK-MB activity was found in 23%(9/39) of the patients, P >0.05 ;there was significant difference between the incidence of elevation of cTnT and CK-MB, P <0.05; there were significant difference between all the elevation of cTnT in the different time alter the procedure and before the procedure( P <0.05 ); but for CK-MB, there was no significant difference( P >0.05).Conclusions:The present study indicates that serum cTnT is a sensitive and specific biomarker for myocardial cell damage, there is important significance to diagnose and predict the prognosis of patients with minor myocardial damage.
