应用彩色多普勒超声诊断隐蔽型神经母细胞瘤

【摘要】 目的应用彩色多普勒超声对腹部不能触及肿块的神经母细胞瘤(隐蔽型)进行检查,协助临床对本病早期诊断、早期治疗。方法该组31例均来自中国医科大学附属第二医院小儿内科、外科、眼科,均经骨髓穿刺镜检后诊断或高度怀疑为神经母细胞瘤,并均做彩色多普勒超声检查,寻找原发病灶。结果该组31例中23例检出实质性肿块,22例位于肾上腺或腹后壁,1例位于后纵隔。半数以上的肿块可显示少数点状或短杆棒状彩色血流信号,呈动脉血流频谱。其中19例行手术治疗,超声与手术结果对照,符合者17例,诊断符合率为89.5%。结论彩色多普勒超声可协助临床对隐蔽型神经母细胞瘤作出诊断以及手术前确定原发肿瘤的部位。

【Abstract】 Objective: To evaluate the value of color Doppler ultrasonography in diagnosis of occult neuroblastoma which intra-abominal mass can not be palpated. Methods: All thirty-one cases who were confirmed or highly suspected as neuroblastoma by bone marrow puncture from Department of Pediatric, Pediatric Surgery and Ophthalmology were performed using color Doppler ultrasonography to search for the primary sites. Results: Solid masses were showed in 23 of 31 cases. Among 23 cases, the masses were located in the adrenal region or retro-peritoneal mediastinum. Dot-like or short sticky color blood flow signals which revealed arterial spectrum were showed in more than half of the masses. 19 cases were received surgical operations. Compared to the surgical results, 17 of 19 cases coincided with the findings of ultrasonography. The coincidence rate was 89.5%. Conclusions: Color Doppler ultrasonography is helpful in diagnosing occult neuroblastoma, it can also help to determine the primary sites of the mass before the surgical operations.
