新斯的明对硬膜外麻醉低血压的影响

【摘要】 目的 :观察新斯的明对硬膜外麻醉血压和心率的影响。方法 :选择 ASA ~ 级的拟行下腹部手术的患者5 0例 ,随机分为对照组 (n=2 5 )和观察组 (n=2 5 )。对照组经硬膜外腔注射 2 %利多卡因 15 ml加生理盐水 1.5 ml,观察组经硬膜外腔注射 2 %利多卡因 15 ml加新斯的明 1.5 mg(1.5 ml) ,观察注药后血压和心率的变化。结果 :麻醉后 ,对照组血压明显降低 ,心率明显减慢 ,观察组血压变化不明显 ,但心率减慢非常明显 ,与对照组相比 ,观察组血压明显高 ,但心率却明显慢。结论 :新斯的明对硬膜外麻醉低血压有预防作用 ,但存在心动过缓的副作用。

【Abstract】 Objective:To observe clinically the prophylactic effect of epidural neostigmine on hypotension induced by epiduralanesthesia.Methods:50 adult ASAⅠ~Ⅱ patients scheduled for electivelower abdomen operation under epidural anesthesia were enrolled in this study,and randomly divided into two groups:control group and test group. All patients were anesthetized by epidural block with 2% Lidocaine 15 ml at L 2~3.Neostigmine 1.5 mg were added into Lidocaine respecttively only in test group. Arterial bloodpressure and heart rate were observed and recorded from anesthesia to epiural block. Ringer′s solution was infused at rate of 10 ml/(kg·h) and no auxillary drugs were administered during observation.Results:After epiural block,systolic arterial pressure,diastolic arterial pressure MAP and heart rate decreased significantly in control group.However,there was no significant change in test group. Compared to control group, the systolic arterial pressure,diastolic arterial pressure MAP were higher and heart rate was lower significantly in test group.Conclusion:Epidural neostigmine can lessen hypotension induced by epidural anesthesia. However, must pay attention to the side-effect of sinus bradycardia.
