不切断Roux-en-Y食管空肠吻合空肠袢肌电活动的改变

【摘要】 目的研究不切断Roux-en-Y食管空肠吻合法对Roux-en-Y滞留综合征的预防作用。方法普通健康幼猪14只,随机分为不切断Roux-en-Y食管空肠吻合组(实验组)和Roux-en-Y食管空肠吻合组(对照组)。术后1周检测空肠袢平滑肌肌电。结果慢波频率:实验组8.4cpm,对照组6.1cpm;慢波振幅:实验组2.31mV,对照组0.66mV;峰电发生率:实验组11.7%,对照组7.4%;峰电振幅:实验组0.58mV,对照组0.31mV;实验组平滑肌肌电均显著高于对照组(P<0.01)。实验组MMCⅢ相时程为6.9min,较对照组的4.2min明显延长(P<0.01),MMC周期实验组(43.3min)较对照组(56.2min)明显缩短(P<0.01)。结论不切断Roux-en-Y食管空肠吻合法可有效改善Roux-en-Y滞留综合征。

【Abstract】 Objective To investigate the effect of “uncut”Roux en Y esophagojejunostomy on preventing Roux en Y stasis syndrome. Methods Fourteen healthy young pigs were randomly divided into 2 groups, and received “uncut”Roux en Y esophagojejunostomy(experimental group), and traditional Roux en Y esophagojejunostomy(control group). Electromyography of jejunal limb was recorded 1 week after operation and compared between two groups. Results Compared with the control group, the frequency of slow wave (8.4 cpm vs. 6.1 cpm),amplitude of slow wave (2.31 mV vs. 0.66 mV),incidence rate of spike potential (11.7%vs. 7.4%) and amplitude of spike potential (0.58 mV vs. 0.31 mV) in experimental group were significantly higher. The duration of MMC phase Ⅲin experimental group was 6.9 min, markedly longer than that in the control group (4.2 min, P< 0.01). The interval of MMC cycle was 43.3 min,significantly shortened as compared to 56.2 min in the control group. Conclusion “Uncut”Roux en Y esophagojejunostomy can effectively relieve the symptoms of Roux en Y stasis syndrome.
