HPV16感染对食管癌及P16、P53基因表达的影响
Effect of HPV16 infection on the esophageal carcinoma and the gene expressions of P53 and P16
【摘要】 目的 通过研究食管癌的HPV16感染阳性率及抑癌基因P16、P5 3表达的影响 ,探讨HPV16感染在食管癌病因学的作用。方法 取食管癌手术切除标本 30例及内镜下取慢性食管炎并非典型增生标本和正常食管黏膜标本各 30例 ,应用PCR方法检测各标本HPV16的感染阳性率 ,并用免疫组织化学方法分别检测各组中的P5 3和P16基因突变率。结果 食管癌组HPV16感染阳性率 4 6 6 7%显著高于癌前病变组 2 3 33%和正常组 3 33% (P <0 0 5 ) ,癌前病变组与正常组的HPV16感染率比较差异有显著性 (P <0 0 5 )。食管癌组中HPV16阳性的P5 3、P16突变率 85 71%、78 5 7%显著高于HPV16阴性的P5 3、P16突变率 5 0 0 %、37 5 % (P <0 0 5 ) ,在癌前病变组HPV16阳性组与阴性组的比较差异无显著性 ,在正常对照组中未发现有突变。结论 HPV16感染在粤西食管癌的发病中具有明显的相关性 ,它可能通过引起P5 3、P16突变而发挥作用
【Abstract】 Objective To investigate the role of human papilloma virus16(HPV16) in etiology of esophageal carcinoma by study the infectious rate of HPV16 and positive expression rate of P53 and P16.Methods Three groups specimens from operations of 30 cases esophageal carcinoma and endoscopies of 30 cases atypical hyperplasia in chronic esophagitis and 30 cases normal were detected the HPV16 infectious rate by PCR. Then the mutation rates of gene P53 and P16 of each group were inspected respectively by immunohistochemistry. Results The HPV16 infectious rate of esophageal carcinoma group 46 67% is significantly higher than those of precancerous lesion group 23 33% and normal group 3 33%( P <0 05). There is a notable difference of HPV16 infectious rate between precancerous lesion group and normal group( P <0 05). The P53 and P16 gene mutation rates of esophageal carcinoma group which are HPV16 positive are significantly higher than those of HPV16 negative. It had no marked difference between those are HPV16 positive and those are HPV16 negative in the precancerous lesion group. The P53 and P16 gene mutation hadn’t detected in normal group.Conclusion The results support that HPV16 play a important role in the pathognesis of esophageal carcinoma probably by causing the gene mutation of P53 and P16.
