【摘要】 目的　探讨经蝶入路治疗颅咽管瘤的适应证、疗效和并发症。方法　采用经蝶入路治疗 18例颅咽管瘤患者 ,术前行CT或MR检查 ,肿瘤直径 2～ 7cm ,其中Samii分型Ⅰ级 5例 ,Ⅱ～Ⅳ级 13例。结果　肿瘤全切除 8例 ,次全切除 6例 ,大部分切除 4例 ,无手术死亡 ,术中脑脊液漏 3例 ,术后尿崩 6例 ,出院时恢复。结论　经蝶入路既适用于鞍内型颅咽管瘤 ,也适用于部分鞍内 鞍上型颅咽管瘤 ,疗效较好 ,并发症发生率低更多还原

【Abstract】 Objective The goal of this study was to assess the outcome of transsphenoidal microsurgical treatment in 18 patients with craniopharyngiomas. Methods The transsphenoidal microsurgery was performed in 18 patients with craniopharyngiomas. The maximal diameters of the tumors are 2 to 7 centimetre on MRI or CT images. According to the radiological findings, Samii Classification was used to classify the tumors into grade Ⅰ in 5 cases and grade Ⅱ to Ⅳ in 13 cases. Results Complete removal of the tumors was accomplished in 8 patients, subtotal removal in 6 patients, and partial removal in 4 patients. No death occurred. An intraoperative cerebrospinal fluid(CSF) leak was encountered in 3 cases. Central diabetes insipidus occurred in 6 patients postoperatively. Conclusions Transsphenoidal microsurgery is an effective and safe method for the treatment of intra-sellar and some of the supra-sellar craniopharyngiomas.更多还原
