小儿病毒性脑炎/脑膜炎脑脊液神经元烯醇化酶及S-100β蛋白检测的临床意义
Study on the changes of cerebrospinal fluid (CSF) neuron-specific enolase and S-100β protein in children with viral encephalitis/ meningitis
【摘要】 目的 观察病毒性脑炎 /脑膜炎患儿急性期及恢复期脑脊液神经元烯醇化酶 (NSE)、S 10 0 β蛋白水平的变化 ,并探讨其临床意义。方法 1997年 1月至 2 0 0 3年 3月兰州医学院第一附属医院 6 8例病毒性脑炎 /脑膜炎患儿作为观察组 ,对照组选择同期住院的非中枢神经系统疾病患儿 2 5例。NSE采用竞争性放射免疫分析法测定 ,S 10 0 β采用酶联免疫双抗体夹心法测定。 结果 急性期单纯疮疹病毒 Ⅰ (HSV Ⅰ )、HSV Ⅱ、EBV感染患儿脑脊液NSE及S 10 0 β蛋白质量浓度较对照组明显增高 (P <0 0 1) ,柯萨奇B组病毒、埃可病毒感染患儿脑脊液NSE及S 10 0 β蛋白质量浓度较对照组增高 (P <0 0 5 ) ,且NSE及S 10 0 β蛋白质量浓度与惊厥发作次数、意识障碍程度、头颅CT脑组织低密度范围呈正相关 (P <0 0 1或P <0 0 5 )。恢复期HSV I、HSV II、EBV感染患儿脑脊液NSE及S 10 0 β蛋白质量浓度仍较对照组增高 (P <0 0 1,P <0 0 5 ) ,且NSE、S 10 0 β蛋白质量浓度与预后程度呈负相关 (P <0 0 1,P <0 0 5 ) ,特别是与恢复期脑脊液NSE及S 10 0 β蛋白质量浓度关系密切。急性脑炎患儿在急性期及恢复期脑脊液NSE及S 10 0 β蛋白质量浓度较对照组明显增高 ,急性脑膜炎急性期除HSV Ⅰ、EBV感染患儿增高外 ,其余与对照
【Abstract】 Objective To investigate the changes of cerebrospinal fluid(CSF) neuron-specific enolase and S-100β protein in children with viral encephalitis/ meningitis and to explore their clinical value.68 children with viral encephalitis/ meningitis were observed,and the control group induded 25 children without central neural system disease.Methods The concentration of neuron-specific enolase and S-100β protein in CSF were measured by immunoradioassays and enzyme linked immunosorbent test.Results In the acute phase,the levels of CSF neuron-specific enolase and S-100β protein were obviously increased in children with viral encephalitis/ meningitis caused by HSV-I、HSV-II、EBV compared with the control group.The concentration of CSF neuron-specific enolase and S-100β protein in children with viral encephalitis/ meningitis infected by CoxBV and ECHO were higher than that of the control group.There was a positive correlation between the concentrations of neuron-specific enolase and S-100β protein in CSF and the seizure times of convulsion,the degree of consciousness handicap and the low density scope of brain tissue in cranial CT(P<0 01 or P<0 05).In the convalescent phase,the levels of CSF neuron-specific enolase and S-100β protein in children with viral encephalitis/ meningitis caused by HSV-Ⅰ,HSV-Ⅱ,EBV were still higher than that of the control group.There was a negative correlation between the levels of neuron-specific enolase and S-100 β protein CSF and the prognosis(P<0 01,P<0 05).In all the 68 cases,the rate infected by enteroviruses (CoxBV,ECHO) was 58 82%,the children presented with acute meningitis,the prognosis was better and the recovery rate was 64 71%; the rate infected by herpesviruses (HSV-Ⅰ,HSV-Ⅱ,EBV) was 25%,the children presented with acute encephalitis syndrome,the state was severe,the prognosis was bad and the recovery rate was only 1 47%.Conclusion The levels of neuron-specific enolase and S-100β protein in CSF in children with viral encephalitis were significantly higher than the control group and may be related to the degree of brain injuy
