【摘要】 目的 :通过检测大肠癌组织内血管内皮生长因子 (VEGF)的蛋白表达及患者术前血清CEA值 ,探讨它们与大肠癌的病理学特征及预后的相关性。方法 :采用免疫组化Envision方法 ,对 78例手术切除大肠癌标本的癌灶、癌旁及正常大肠组织进行血管标记和染色 ,并进行定位观察 ,检测其VEGF表达 ,同时回顾分析血清CEA值变化。结果 :78例大肠癌患者中 ,4 8例患者 ( 6 1.2 % )VEGF蛋白表达阳性 ,CEA阳性患者 35例 ( 4 4.7% )。它们与肿瘤浸润深度、淋巴结、血行转移及Dukes分期呈明显相关性 (P <0 .0 5 ) ;而与肿瘤大小、组织学分型、生长方式无关 (P >0 .0 5 )。而且VEGF阳性组与阴性组 5a生存率分别为 6 6 .8%、86 .7% ,差异显著 (P <0 .0 5 ) ;CEA阳性组与阴性组 5a生存率亦有明显区别 (P <0 .0 5 ) ;VEGF + /CEA +组 5a生存率最低。结论 :VEGF和CEA与大肠癌浸润转移 ,分期及生存率呈明显相关性 ,VEGF和CEA联合检测 ,对大肠癌预后的判断更具有实际意义。更多还原

【Abstract】 Objective:Elucidate the expression of VEGF and CEA in colorectal carcinoma and their association.Methods:The expression of VEGF was examined by immunohistochemical staining in 78 specimens of colorectal carcinoma and CEA value of these cases were reviewed.Results:The positive rate of VEGF was 61.2%,The CEA-positive rate was 44.7%,VEGF and CEA have no relations with tumor size、pathohistological typing、growth pattern.but they were significantly related with the infiltration growth、lymph node as avellas blood metastatsis and Dukes pattern( P <0.05).The 5-year survival rate for VEGF+/VEGF-、CEA+/CEA- patients was 66.8%/86.7% 、68.6%/90.2%,the difference was significant;The 5-year survival rate for VEGF+/CEA+ patients was 45.2%,which was significantly lower than that for VEGF+/CEA-、VEGF-/CEA+ and VEGF-/CEA-.Conclusions:VEGF and CEA were significantly related with tumor progression and poorer prognosis in colorectal carcinoma.But the combination criterion of VEGF and serum CEA value should be a more useful prognosticator,the prognosis of VEGF+/CEA+patients are very poor for colorectal carcinoma.更多还原
