Smad4蛋白和TGF-β1及TβRⅡ在胆管癌中表达的研究

【摘要】 目的 探讨胆管癌组织中Smad4蛋白和TGF β1 及TβRII表达的相互关系及其在胆管癌发生发展中的可能作用机制。方法 应用免疫组化SABC法检测 49例胆管癌及 8例正常胆管组织中Smad4蛋白和TGF β1 及TβRII的表达。结果  49例胆管癌组织Smad4蛋白 ,TGF β1 和TβRII蛋白阳性率分别为 40 .82 % ( 2 0 /4 9) ,71.43 % ( 3 5 /4 9)和 3 6.73 % ( 18/4 9) ;而正常胆管组织阳性率分别为87.5 0 % ( 7/8) ,12 .5 0 % ( 1/8)和 10 0 % ( 8/8)。III~IV期胆管癌的TGF β1 阳性率明显高于I~II期 ;有转移者高于无转移者。I~II期患者TβRII阳性率则明显高于III~IV期 ( P <0 .0 5 )。TGF β1 及其II型受体的共同表达与胆管癌临床病理分期有关 ( P <0 .0 5 )。中下段胆管癌Smad4蛋白表达明显降低 (P <0 .0 5 )。结论 TGF β1 过度表达可能加速了胆管癌的进程 ;Smad4蛋白表达与胆管癌发生的部位有关。

【Abstract】 Objective To investigate the relationship between the expression of the Smad4,TGF-β1 and TβR II and the possible mechanisms in the development of cholangiocarcinoma. Methods Immunohistochemical SABC method was used to examine the expression of Smad4,TGF-β1 and TβR II in 49 cases of cholangiocarcinoma and 8 nomal common bile duct.Results The expression rates of Smad4,TGF-β1 and TβR II in cholangiocarcinoma were (40.82)%(20/49),71.43%(35/49) and 36.73%(18/49) respectively; in normal common bile duct tissues were 87.50(7/8),12.50%(1/8) and 100%(8/8) respectively. There was a significant relationship between the (expression) of TGF-β1 and clinical stage, and tumor metastasis (P<0.05) . TβR II positivity was significantly (lower) in cholangiocarcinoma of clinical stage III-IV than that of stageI-II. The co-expression of TGF-β1 and TβR II was negatively related with clinical stage(P <0.05). Smad4 was less expressed in the distal bile duct (carcinormas) than in the proximal bile duct carcinormas(P<0.01).Conclusions The overexpression of TGF-β1 may play an important role in promoting progression of cholangiocarcinoma . The expression of Smad4 is closely (related) to the anatomical position of cholangiocarcinoma.
