【摘要】 社区健康档案的利用 ,是反映全科医疗连续性、综合性、协调性服务的重要特点 ,健康档案是对病人、亚健康人群一生中所有医疗资料的记录。通过健康档案的利用经每年一次区域健康人群体检或经全科医生门诊可以定期对慢性病实行干预计划。通过健康档案和监测资料可以分析高危人群的危险因素 ,确定干预计划 ,设计针对性干预方案 ,跟踪指导。更多还原

【Abstract】 How to take use of community health reflects the important characteristics of General Medicine, which are continuity, synthetic, and harmony. Health records include all medical material of the patients and inferior health population in their life. The way of using health records is to intervene chronic diseases by plan at interval through general doctors out-patient or physical-exam for healthy population in the community annually. Through healthy records and supervising material, we can analyze the dangerous factors of high-risk population, then make intervening plan, design sharp programme and direct their life.更多还原
