7040例新生儿听力筛查结果分析

【摘要】 目的 :评价畸变产物耳声发射在新生儿听力筛查中的应用价值 ,获得新生儿听力损失发病的初步资料。方法 :新生儿出生 3~ 4天及出生 4 2天分别用畸变产物耳声发射仪 (DPOAE)进行普遍听力初筛复筛 ,复筛未通过者脑干诱发电位 (ABR)诊断 ,异常者在 6个月时再次复查 ABR,两次 ABR未通过者为听力损失。结果 :住院期间筛查新生儿 70 4 0例 ,1次通过 5 76 7例 ,通过率 81.9% ,4 2天再次复查 5 731例 ,通过 5 6 0 4例 ,通过率 97.8%。未通过者 3个月内 ABR检查 114例 ,有 4 8例未通过 ,至 6个月时再次复查 ABR,有 36例转为正常 ,最后确诊不同程度听力损失 12例 ,听力损失发病率 1.70‰。结论 :DPOAE是目前新生儿听力筛查理想的检测方法 ,住院期间普遍听力初筛和出生4 2天普遍复筛有利降低筛查假阴性和假阳性 ,两次 ABR确诊有利提高诊断准确性。

【Abstract】 Objective To seek the practical value of distortion product otoacoustic emissions(DPOAE) for evaluating in the universal newborn hearing screening.Method All newborns received hearing screening test by DPOAE in 72-96 hours and rescreened 42 days after they were born.Those infants who failed to pass the rescreening test received auditory brainstem response (ABR) test in three months.Those infants who failed in the ABR test received ABR test again when they were in six months.Results 5767 (81.9%,5 767/7040) newborns passed the DPOAE screening in the hospital. 5731 infants received DPOAE rescreening test and 5604 (97.8%,5 604/5731) infants pass. 46 of 114 infants receiving ABR test failed to pass at their three month old. but 34 infants passed ABR test in their six months old. 12 infants had been confirmed hearing loss.Conclusion The DPOAE is an ideal methods of present auditory screen in early neonates .Two stage screening combined can reduce OAE false negative and false positive and increased the accuracy of diagnosis.
