【摘要】 目的　采用 6min步行试验 ,观察充血性心力衰竭 (CHF)患者运动前后血清肿瘤坏死因子α(TNFα)、白细胞介素 1 0 (IL - 1 0 )水平的变化。方法　 34例CHF患者和 2 2例健康对照者进行了 6min步行试验 (6 -MWT) ,于运动前后采取静脉血 ,使用双抗体夹心ABC -ELISA法测量血清IL - 1 0和TNF -α的浓度。结果　CHF组基础状态的血清TNF -α水平显著高于对照组 (P <0 .0 1 ) ,而IL- 1 0水平和 6min步行距离 (6 -MWTD)均低于对照组 ,但IL - 1 0的变化差异无统计学意义 (P >0 .0 5)。与运动前相比 ,对照组运动后即刻TNF -α水平无统计学意义 (P >0 .0 5) ,但IL - 1 0水平显著性升高 (P <0 .0 5) ;CHF组TNF -α水平运动后即刻呈明显升高(P <0 .0 1 ) ,而IL - 1 0水平运动后即刻并无统计学意义的升高 (P >0 .0 5)。结论　CHF患者较正常人运动耐量下降 ,一次 6min步行运动使CHF患者炎症性细胞因子水平进一步增加 ,抗炎症性细胞因子水平不增加 ,从而加重了细胞因子之间的网络调控紊乱。更多还原

【Abstract】 Objective To study the effect of 6 minute walking test(6-MWT) on the levels of serum interleukin-10(IL-10) and tumor necrosis factor-α(TNF-α) in patients with congestive heart failure(CHF). Methods 34 CHF patients and 22 healthy subjects underwent 6-MWT.The blood samples were taken before and immediately after 6-MWT from all subjects.Serum IL-10 and TNF-α levels were assessed with a sandwich ELISA assay. Results The levels of serum TNF-α were significantly higher(P<0.01),IL-10 levels were significantly lower,and 6-MWT distances were significantly shorter(P<0.01) in CHF patients than those in healthy subjects at rest,but IL-10 levels between CHF patients and healthy subjects had no significantly change(P>0.05).Compared with those before 6-MWT,Serum TNF-α levels immediately after 6-MWT in healthy subjects had no significantly change(P>0.05),but IL-10 levels were significantly higher(P<0.05);Serum TNF-α levels immediately after 6-MWT in CHF patients were obviously higher(P<0.01),but IL-10 levels had no significantly change(P>05). Conclusion Exercise intolerance in CHF patients is decreased.a 6-MWT make inflammatory cytokine levels significantly raised and anti-inflammatory cytokine levels relatively lowered in CHF patients.Therefore,a 6-MWT may aggravate the imbalance cytokine network control in CHF patient.更多还原
