【摘要】 目的　探讨空气灌肠法治疗急性肠套叠不成功的原因,分析空气灌肠过程中容易出现的问题,以期提高使用该方法治疗肠套叠的成功率.方法　回顾性分析17a来共85例小儿肠套叠手术病例,找出空气灌肠失败的原因.结果　空气灌肠不成功与病儿发病及就诊时间、肠套叠类型、患儿一般情况、医生的临床经验等因素密切相关.结论　认为有部分患者如果在空气灌肠时注意以下几点就有可能避免一些不必要的手术.1)选择合适的患者,病程时间的长短仅作为参考,主要还应根据患者的全身状态、腹部透视的情况而定.2)使用空气灌肠机前一定要仔细检查机器功能是否处于正常状态.3)气囊充气要足够,并保证气囊与肛门括约肌紧密贴合.4)操作过程中应注意压力及肠管充气情况的变化,随时调整.5)配合手法按摩.6)合适病例,可重复操作1～2次.更多还原

【Abstract】 ObjectiveTo increase success rate of airenema reduction to treat intussusception through analyzing frequent problem and failure reason of airenema.MethodsWe retrospectively analyzed 85 cases with intussusception to find the failure reason of airenema reduction in recent 17 years.ResultsSuccess rate of airenema reduction has a close relationship with preliminary diagnosis time, category, (patient’s) general condition and clinical experience of surgeon.ConclusionWe considered that the following factors might help to increase success rate of airenema reduction and avoide unnecessary operation. 1)It is very important to choose suitable patient according to the general condition and the result of abdominal fluoroscopy. Length of morbidity time can only be treated as a reference factor. 2)The equipment function should be examined carefully before procedure. 3)Air pocket of anal canal should be charged sufficiently and approached tightly with anal sphincter. 4)Change of air pressure and charge status of intestine should be closely observed during the procedure. The procedure should be regulated according to the condition. 5)Maneuver chirapsia of abdomen is a helpful auxiliary therapy. 6)One or two repetitive procedure is acceptable in some suitable cases.更多还原
