产超广谱β-内酰胺酶肺炎克雷伯氏菌的耐药性监测

【摘要】 目的 对肺炎克雷伯氏菌进行超广谱 β-内酰胺酶检测及耐药性测定 ,为产 ESBL s细菌的监控和治疗提供依据。方法 对临床分离的肺炎克雷伯氏菌采用 K- B纸片扩散法 ,严格按照美国 NCCL S制定的标准 ,进行 ESBL s检测并对 19种抗生素的耐药性测定。结果 在 4 1株肺炎克雷伯氏菌中 ,检出 16株产ESBL s,阳性率为 39.0 %。ESBL s阳性株除对亚胺培南、头孢哌酮 /舒巴坦高度敏感外 ,对其它抗生素的耐药性明显高于 ESBL s阴性株。结论 产 ESBL s的肺炎克雷伯氏菌在临床分离株中阳性率较高 ,且对多种抗生素耐药 ,应加强对 ESBL s的检测和其感染者的治疗。

【Abstract】 Objective To survey ESBLs and drug resistance of Klebsiella pneumoniae and guide the control and treatment of ESBL producing bacteria. Methods Forty-one strains of Klebsiella pneumoniae were isolated from clinical specimens. ESBL-producing strains were confirmed by the NCCLS phenotypic confirmatory test, the antibacterial susceptibility testing on 19 kinds of antibiotics was performed by K-B method. Results Among 41 strains of Klebsiella pneumoniae , ESBL producing bacteria accounted for 16 (39.0%). ESBL producing strains were susceptible to imipenem and cefoprazone/sulbactam, showing more resistant to other antibiotics than ESBLs negative strains. Conclusion The incidence of ESBLs in Klebsiella pneumoniae was rather high, and these strains were usually resistant to conventional antibiotics.
