β-catenin在结直肠肿瘤中的异常表达及其意义
Expression of β-catenin in human colorectal adenoma and carcinoma
【摘要】 目的 :研究 β- catenin在结直肠肿瘤中异常表达的意义。方法 :应用免疫组织化学 SP法 ,检测 β- catenin在腺癌患者正常黏膜、癌旁黏膜、腺癌组织中及其在腺瘤组织中的表达情况。结果 :在正常黏膜和癌旁黏膜中 ,β- catenin主要定位于细胞膜 (胞膜阳性率分别为 94 .6 %和 86 .5 % ) ,并有部分在胞浆中表达 (胞浆阳性率分别为 38.7%和5 5 .0 % ) ,胞核均为阴性 ;在腺瘤与腺癌中 ,其阳性染色主要定位于细胞浆 (胞浆阳性率分别为 85 .1%和 93.7% ) ,并有部分细胞核阳性 (分别为 12 .8%和 2 3.4 % ) ,胞膜阳性表达程度显著低于正常和癌旁黏膜 (P<0 .0 5 ) ,胞浆、胞核的表达高于正常和癌旁黏膜 (P<0 .0 5 ) ;而高、中分化腺癌在细胞浆的阳性表达率显著高于低分化腺癌 (分别为 10 0 %、95 .5 %、6 8.8% ,P<0 .0 5 ) ;在腺癌组织中β- catenin的胞浆阳性表达程度与腺癌的 Dukes分期相关 ,A期表达程度显著低于 B/C期。结论 :β- catenin在结直肠腺瘤、腺癌中胞浆的表达高于正常黏膜及癌旁黏膜 ;其表达与结直肠癌的分化程度及 Dukes分期相关 ,因此 ,β- catenin表达检测可以作为预后判断的参考指标之一。
【Abstract】 Objective: To investigate the expression of β-catenin and its significance in colorectal neoplasms. Methods: Tissue specimens of normal colorectal mucosa, mucosa adjacent to carcinoma, colorectal adenoma and adenocarcinoma were examined for β-catenin with immunohistochemistry. Results: β-catenin was mainly expressed in the cytomembrane of normal mucosa and mucosa adjacent to cancer (the positive rates were 94.6% and 86.5%, respectively) and also in the cytoplasm (the positive rates were 38.7% and 55.0%, respectively), while its expression was negative in the cell nucleus. In adenoma and adenocarcinoma, β-catenin was mainly expressed in the cytoplasm (the positive rates were 85.1% and 93.7%, respectively) and partially in the cell nucleus (the positive rates were 12.8% and 23.4%, respectively). Compared with normal mucosa and mucosa adjacent to cancer, the expression of β-catenin in the cytomembrane of adenoma and adenocarcinoma was significantly lower (P<0.05), while its expression in the cytoplasm and cell nucleus of adenoma and adenocarcinoma was significantly higher (P<0.05). The positive rates of cytoplasm in highly- and moderately-differentiated adenocarcinoma were significantly higher than that in poorly-differentiated adenocarcinoma (the positive rates were 100%, 95.5% and 68.8%, respectively).β-catenin expression rate in cytoplasm was correlated with Dukes’ stages of adenocarcinoma, which was significantly lower in stage A than in stage B/C. Conclusion: The expression of β-catenin is significantly correlated with differentiation and Dukes’ stages of colorectal carcinoma and it can be used as an indicator for the prognosis of colorectal carcinoma.
