【摘要】 目的 :探讨对于脊柱肿瘤一期全脊椎切除及脊柱稳定性重建的手术方法和临床效果。方法 :对 12例脊柱肿瘤行前、后路或前、后联合入路Ⅰ期全脊椎切除、脊髓减压 ,椎间植骨融合、内固定术。结果 :术后 11例获得随访 ,按照Frankel分级较术前均有 1级以上提高 ,所有患者局部疼痛症状消失。术后复查平均植骨融合时间 3个月。1例脊索瘤术后 1年复发 ,1例巨细胞瘤术后 9个月复发。结论 :针对脊柱肿瘤的性质、部位 ,通过不同的手术入路行一期全脊椎切除、植骨融合内固定 ,彻底切除肿瘤 ,重建脊柱稳定性并体现了个体化的治疗方向更多还原

【Abstract】 Objective:To discuss the surgical approach and clinical effect of total vertebrectomy and spine stability and reconstruction Method:Twelve patients of total vertebral tumor were operated by anterior or posterior approach or the both ways One stage total vertebrectomy,decompression,fixation and intervertebral fusion were performed on the patients with spinal tumor Result:Eleven of 12 patients were followed up After decompression,neurological function,evaluated as Frankel grade,more than 1 grade rising was obtained in all the patients At the same time,pain relief was obtained in all the patients The mean time of fusion were 3 months One patient with chordoma recurred after 1 year of operation Another patient with giant cell tumor recurred after 9 months of operation Conclusion:The surgical treatment of spinal tumor depend on the tumor type,location in the spine There are three approaches for one stage total vertebrectomy followed by fixation and intervertebral fusion,including of anterior,simultaneous anterior posterior and posterior approach更多还原
