氯胺酮芬太尼静脉复合麻醉在婴幼儿短小手术中的应用

【摘要】 目的 :比较氯胺酮芬太尼静脉复合麻醉和氯胺酮静脉麻醉应用于婴幼儿短小手术中的麻醉效果。方法 :将 4 0例手术患儿随机分为两组 ,Ⅰ组为氯胺酮芬太尼静脉复合麻醉组 (2 0例 ) ,Ⅱ组为氯胺酮静脉麻醉组 (2 0例 )。术中连续监测HR、MAP、RR、SPO2 并分别记录 ,观察患儿苏醒时间及有无并发症。结果 :Ⅰ组患儿切皮时、手术 5min、10min及术毕时的心率、平均动脉压与诱导前相比无明显变化 (P >0 .0 5 ) ,术中未出现低氧血症 ,呼吸道分泌物少 ,苏醒后有 1例哭闹 ,余醒时平稳 ;与Ⅰ组相比 ,Ⅱ组切皮时、手术 5min、10min及术毕时的心率、平均动脉压均明显增高 ,与诱导前相比 ,有明显变化 (P <0 .0 5 ) ,术中有 1例出现呼吸抑制 ,经面罩吸氧后缓解 ,有 4例呼吸道分泌物较多 ,所有患儿苏醒时间均长于Ⅰ组 ,且有哭闹现象。结论 :在婴幼儿短小手术中应用氯胺酮芬太尼静脉复合麻醉优于氯胺酮静脉麻醉。

【Abstract】 Objective:To compare the anesthetic effects between the combined sketamine-fentanyl intravenous anesthesia and only ketamine intravenous anesthesia on children’s short operation. Methods: Forty patients were divided into two groups.In groupⅠthe patients received ketamine-fentanyl combined intravenors anesthesia (20 cases), in groupⅡthe patients received ketamine intravenous anesthesia (20 cases). Heart rate (HR), MAP, RR,SPO 2 and revival time were monitored.measured. and recorded during the operation.Results:The changes of HR and MAP had no significant difference in group Ⅰ (P>0.05) . There was not hypoxemia and much respiratory secretion.One patient cried, the others were safe. Comparing with group Ⅰ,the rate of the HR and MAP was increased significantly in groupⅡ(P<0.05).One patient’s respiration was restrained. and got better after aspirating oxygen .Four patients’respiratory secretion was more. All the patients’ revival time was longer than that of group Ⅰ,and they cried after revival. Conclusion:The anesthetic effect of the combined ketamine-fentanyl intravenous anesthesia is better than that of ketamine invravenous anesthesia on pediatric short operation.
