腹腔镜胆囊切除术对呼吸循环的影响及护理

【摘要】 目的 观察全麻下腹腔镜胆囊切除术人工CO2 气腹后患者呼吸和循环的变化。方法 选择ASAⅠ~Ⅱ级 ,静 吸复合全麻下行腹腔镜胆囊切除术者 34例 ,观察记录CO2 气腹前后血压、心率、动脉血氧饱和度和呼气终末CO2 分压等参数。结果 气腹后 5min时患者收缩压、舒张压、平均动脉压、心率、呼气终末CO2 分压比气腹前明显升高 (P <0 0 5 ) ,动脉血氧饱和度明显下降 (P <0 0 5 )。结论 全麻下腹腔镜胆囊切除术 ,人工CO2 气腹后引起患者血压、心率、呼气终末CO2 分压升高和动脉血氧饱和度下降 ,应加强术中呼吸循环的监测 ,以保证患者安全度过围手术期

【Abstract】 Objective To investigate the effect of carbon dioxide insufflation on respiration and circulation during laparoscopic cholecystectomy(LC).Methods Thirty four patients with grade Ⅰ~Ⅱ ASA undergoing LC were selected.The NIBP,HR,SpO 2,P ET CO 2 parameters were measured before and after pneumoperitoneum.Results Compared with those before pneumoperitoneum,the SBP,DBP,MAP,HR,P ET CO 2 increased significantly(P<0.05),SpO 2 declined significatly(P<0.05).Conclusion Following the carbon dioxide insufflation during LC with general anesthesia,the blood pressure,heart rate,end tidal carbon dioxide tension elerate with decline of SpO 2.Therefore:the monitor of respiration and circulation must be emphasized during operation and nursing in order that pationt might be safe during perioperative period.
