湖北省血吸虫病3次抽样调查疫情对比分析
COMPARISON OF ENDEMIC SITUATION OF SCHISTOSOMIASIS FROM THREE SAMPLING SURVEYS IN HUBEI
【摘要】 目的 掌握湖北省血吸虫病疫情变化情况 ,为制定防治规划提供科学依据。 方法 采用分层整群随机抽样法抽取行政村 ,对人、畜进行血吸虫粪检查病 ,用Epi info软件对 3次抽样调查结果进行统计分析。 结果 3次抽样调查居民血吸虫感染率分别为 13 .5 4%、7.19%、4.77% ,病人感染度 (EPG) 1989、1995和 2 0 0 1年算术均数分别为 5 1.97、3 6.85、3 4.5 3 ,人群感染度 (EPG)算术均数分别为 7.0 4、2 .65、1.65 ,耕牛感染率分别为 18.71%、14 .98%、9.80 % ,均呈下降趋势。按年龄分组 ,5 0~ 60岁组病人感染度、人群感染度下降幅度最小 ,且 2 0 0 1年该组病人感染度和人群感染度在各年龄组中最大 ,感染率也较高。各职业中农民、渔民、船民感染率高 ,感染度大 ,且渔民中的病人感染度呈上升趋势。丘陵地区居民感染率 1995年、2 0 0 1年和 1989年相比均上升 ,耕牛感染率也呈上升趋势。 结论 湖北省血吸虫病疫情总体呈下降趋势 ,但高年龄组居民、农民、渔民、船民血吸虫感染仍较严重。丘陵地带的疫情呈上升趋势。
【Abstract】 Objective To understand the changes of endemic situation of schistosomiasis and provide the scientific basis for making the control program in Hubei Province. W Methods Stratifying and clustering random sampling was applied to select administrative villages. Residents and cattle in selected villages were detected by stool examination. Results of stool examination from three sampling surveys (in 1989,1995,2001) were statistically analyzed by Epi info software. W Results In three sampling surveys, the infection rate of residents was 13.54%, 7.19%, 4.77%, respectively, arithmetic mean of patient EPG was 51.97, 36.85, 34.53, respectively, arithmetic mean of population EPG was 7.04, 2.65, 1.65, respectively, the infection rate of cattle was 18.71%, 14.98 %, 9.80%, respectively. All indexes showed decrease trends. In different age groups, the decrease of patient EPG and population EPG in 50-60 age group was minimal, its patient EPG and population EPG were the biggest and its infection rate was high in 2001. In different occupations, the infection rate of farmer, fishermen and boatmen was comparatively higher and their EPG was big. In the meantime, the patient EPG of fishermen increased. The infection rate of residents in hill area in 1995 and 2001 increased as compared with that in 1989. There also was a increase trend in positive rate of cattle in hill area. W Conclusion The schistosomiasis prevalence in Hubei Province was generally decreasing. However, schistosome infection of high age group, fishermen and boatmen was still serious. The schistosomiasis prevalence in hill area increased.
