海绵体神经移植恢复大鼠勃起功能的研究
Cavernous nerve grafts restore erectile function in rats
【摘要】 目的 应用腓肠神经移植替代损伤的双侧海绵体神经恢复大鼠的勃起功能。方法 将 48只雄性SD大鼠随机分为 3组 :神经移植组、神经损伤组及假手术组。 2、4个月后 ,海绵体神经电刺激检测大鼠勃起功能 ,免疫组织化学法检测海绵体内nNOS阳性神经纤维。结果 2个月后神经移植组与神经损伤组大鼠对海绵体神经电刺激均无勃起反应 ,两组海绵体内nNOS阳性神经纤维数目差异无显著性 (P >0 .0 5 ) ;而 4个月后神经移植组大鼠勃起功能较神经损伤组差异有显著性 (P <0 .0 5 ) ,海绵体内nNOS阳性神经纤维数目也显著高于神经损伤组 (P <0 .0 5 ) ,与假手术组差异无显著性 (P >0 .0 5 )。结论 腓肠神经移植替代损伤的双侧海绵体神经可恢复大鼠的勃起功能。
【Abstract】 Objective To investigate whether reconstruction of cavernous nerve can restore erectile function using sural nerve as an interposition nerve graft.Methods Forty-eight male Sprague-Dawley rats (3-4 month old,weighing 300-400 g) were randomly divided into three groups:sham-operated controls (n=16) underwent pelvic exploration without transection of the cavernous nerve;nerve ablation group (n=16) had a 5 mm segment of cavernous nerve excised bilaterally;graft group (n=16) had a 5 mm segment of cavernous nerve excised bilaterally,followed by immediate microsurgical reconstruction with an interposition graft of sural neve.Two and 4 months postoperatively the animals of each group were electrostimulated to the cavernosal nerves to determine potency.Corpora cavernosa were harvested and nNOS-positive nerve fibers were examined by using streptavidin-peroxidase immunohistochemistry techniques (SP method).Results At 2nd month,electrical stimulation produced no erections in the nerve ablation or the graft group and 100% erections in the sham-operated group.There was no significant difference in the nNOS-positive nerve fibers between the nerve ablation group and the graft group.At 4th month,43.75% of the grafted nerves and 62.5% of the grafted rats produced erections upon electrical stimulation,significantly different from those in the nerve ablation group (6.25% and 12.5% respectively).The nNOS-positive nerve fibers in the graft group (82.0±12.4) significantly differed from those in the nerve ablation group (27.3±4.1),and approximated the level of the sham-operated group (94.5±7.4).Conclusion Cavernous nerve grafting in rats can be successful in restoring potency after surgical injury.Application of sural nerve as an interposition nerve graft to man may be indicated.
