绝经后妇女血清降钙素基因相关肽、同型半胱氨酸水平与性激素及冠心病的关系
The relationship between serum calcitonin gene-related peptide, sex hormone, homocysteine and coronary artery disease in postmenopausal women
【摘要】 目的 了解绝经后女性冠心病 (CHD)患者血清降钙素基因相关肽 (CGRP)、同型半胱氨酸 (Hcy)与女性激素水平的关系。方法 用断面调查方法收集经冠状动脉造影确诊的绝经后女性CHD患者 75例、非CHD患者 6 9例及同期年轻女性 6 6例 ,测定血清CGRP、Hcy及性激素水平。 结果CHD组血清Hcy水平 (15 3± 6 5 ) μmol/L高于非CHD组 (10 2± 2 8) μmol/L ,P <0 0 1。CHD组血清CGRP水平 (10 3 6± 5 9 8)ng/L低于非CHD组 (16 4 6± 5 0 7)ng/L ,P <0 0 1。CHD组女性激素水平低于非CHD组 [雌二醇 :(6 7 9± 2 4 4 ) pmol/L比 (91 7± 2 3 0 ) pmol/L ,P <0 0 1;孕酮 :(0 89± 0 4 6 )nmol/L比 (1 11± 0 4 5 )nmol/L ,P <0 0 1]。CHD组患高血压、糖尿病的比例高于非CHD组。CHD组患者年龄高于非CHD组。多因素分析证实 ,Hcy的OR大于 1,并有显著差异 ;CGRP与女性激素的OR小于 1。用前进法观察Hcy、女性激素及CGRP的偏回归系数变化 ,证实Hcy是CHD的独立危险因素 ;CGRP及女性激素是CHD的独立保护因素。结论 Hcy是导致女性CHD的独立危险因素。CGRP是CHD的独立保护因素。女性激素对Hcy及CGRP水平无明显影响
【Abstract】 Objective To investigate the relationship between serum calcitonin gene-related peptide (CGRP), homocysteine (Hcy), sex hormone, and coronary heart disease (CHD) in postmenopausal women. Methods In a cross-sectional study, serum CGRP, estrodiol (E2), progesterone(P)and Hcy levels of 144 postmenopausal women undergoing diagnostic CHD (75 with CHD and 69 without CHD) and 66 healthy young women were measured. Results The occurrence of CHD was correlated with high Hcy level and low CGRP level. The mean serum CGRP level was significantly lower in CHD postmenopausal women than in without CHD ones [(103.6±59.8) ng/L vs (164.6±50.7) ng/L, P<0.01]. The mean serum E2 level was significantly lower in CHD postmenopausal women than in without CHD ones [(67.9±24.4) pmol/L vs (91.7±23.0) pmol/L, P<0.01]. The mean serum P level was significantly lower in CHD than in without CHD postmenopausal women [(0.89±0.46) nmol/L vs (1.11±0.45) nmol/L, P<0.01]. The mean serum Hcy level was significantly higher in CHD than in without CHD postmenopausal women [(15.3±6.5) μmol/L vs (10.2±2.8) μmol/L, P<0.01]. By multivariate logistic regression, the OR of high Hcy level ≥1, P<0.01,that means Hcy is an independent risk factor of CHD. The OR of CGRP, E2 and P were all≤1, indicating that they were independent protective factor. Conclusions Hcy is an independent risk factor of CHD. CGRP, E2 and P are independent protective factors of CHD. There was no relationship beween Hcy, CGRP and E2 and P.
