超声对胡桃夹现象诊断标准的探讨

【摘要】 目的 探讨多普勒超声对胡桃夹现象的诊断标准。方法 测量 3 5例患者平卧位及站立 15min后左肾静脉最窄处和最宽处内径 (a ,b)、血流速度 (Va,Vb) ,及最窄处峰值静脉压差 (ΔP) ,并与对照组进行对比分析。结果 胡桃夹现象患者左肾静脉近端血流速度明显增快 ,b/a增大 ,Va/Vb 增大 ,ΔP升高 ,且平卧位和站立 15min后有显著差别。结论 左肾静脉近端流速明显增快 (站立后 >10 0cm /s) ;平卧位b/a >3 ,站立后b/a >5 ;站立后ΔP >5mmHg(1mmHg =0 .13 3kPa) ,符合其中两项可诊断胡桃夹现象 ,其敏感性、特异性、准确性分别为 10 0 %,94.5 %,96.7%。

【Abstract】 Objective To observe the diagnostic standards of Nut Cracker syndrome with Dopple ultrasound. Methods To measure the proximal restricted and distal unrestricted interdiameter (a,b),and blood flow velocity(V a,V b) as well as the Δ P of stress difference of peak value of the proximal left renal vein in 35 patients with Nut Cracker syndrome in supine position. After standing for 15 minutes,those data were measured again.Statistics analysis was then processed. Results Blood flow of the proximal left renal vein was increased,the ratios of b/a and V a/V b were also increased in the Nut Cracker syndrome patients.Statistics analysis indicted differences between supine and upstanding positions.Conclusions (1)The increased blood flow velocity (>100 cm/s after standing for 15 minutes);(2) the ratio of b/a>3 of left renal vein in supine,and b/a>5 after standing for 15 minutes;(3)after standing for 15 minutes Δ P>5 mm Hg(1 mm Hg= 0.133 kPa).Two of which may be chosen for the diagnostic standards in the Nut Cracker syndrome.The sensitivity,specificity and accuracy are 100%, 94.5 %, 96.7 %,respectively.
