实验性原发性甲状旁腺机能亢进兔血清钙、磷、PTH水平分析
Level of serum calcium, serum phosphorus, and serum parathyroid hormone in early primary hyperparathyroidism in experiment of rabbits
【摘要】 目的 :研究早期原发性甲状旁腺机能亢进 (PHPT)的血清钙、磷、甲状旁腺素 (PTH)诊断价值。方法 :将 6 0只成年白兔随机分为 6组 ,每组 1 0只。实验组和对照组各 30只 ,对照组喂以正常饮食 (Ca∶P ,1∶0 .7) ,实验组喂以高磷饮食 (Ca∶P ,1∶7) ,建立原发性甲状旁腺机能亢进动物模型 ,在喂养前一周及喂养后第 1 ,2 ,3个月时 ,对所有动物做血钙、血磷、血PTH测定 ,并在第 1 ,2 ,3个月末时 ,将对照组与实验组动物各 1 0只处死 ,进行甲状旁腺及肾组织学检查 ,观察甲状旁腺及肾脏的组织学改变。结果 :实验组血PTH水平明显高于对照组 ,血钙水平明显低于对照组 ,而血磷水平变化无显著差异。甲状旁腺组织学检查 ,在 1个月时无变化 ,在 2个月时 ,实验组中的 1 0例中 6例显示轻度增生 ,3个月时 1 0例中 9例轻、中度增生。肾脏组织学检查 ,在 1个月时无变化 ,但在第 2、3个月时发现肾实质内炎性改变 ,并伴钙盐沉积。结论 :血清钙下降 ,血磷无明显变化 ,血PTH水平升高可能比高血钙、低血磷对PHPT的早期诊断更为重要
【Abstract】 Objective: To study the dignostic value of the serum calcium, serum phosphorus, and serum parathyroid hormone(PTH) in the early primary hyperparathyroidism(PHPT) . Methods: Sixty rabbits were divided into 6 groups with 10 in each and received a control diet (Ca∶P,1∶0.7) or a high phosphate diet (Ca∶P, 1∶7 ) to establish model of primary hyperparathyroidism for 1 , 2 , or 3 month intervals. Levels of serum PTH, serum calcium, and serum phosphorus were determined and compared between the controls and experimental animals. The parathyroid and kidneys of all animals received histologic examination. Results: Compared with the control animals, serum PTH levels were elevated at all time intervals in experimental group, whereas serum calcium levels were decreased at all time intervals, but serum phosphorus levels did not change . Histopathologic studies demonstrated no change in parathyroid 1 month later whereas 6 of 10 experimental animals showed mild hyperplasia 2 months later and 9 of 10 showed mild to moderate hyperplasia with gland enlargement 3 months later compared with controls. Histopathologic examination showed no change in kidneys 1 month later but focal parenchymal inflammation with calcium deposition 2 and 3 months later in the experimental groups. Conclusion: Serum calcium levels were decreased, and serum PTH levels were elevated, but serum phosphorus levels did not change in the rabbit model. It might be a reliable diagnostic value for the early PHPT.
