早期应用辛伐他汀对急性冠状动脉综合征血清基质金属蛋白酶及其抑制因子和高敏C-反应蛋白水平的影响

【摘要】 目的 :研究早期应用辛伐他汀对急性冠状动脉综合征患者稳定斑块、减少炎症反应的作用。方法 :采用随机、对照方法 ,将 13 7例急性冠状动脉综合征患者分为他汀治疗组 (辛伐他汀 2 0mg/d ,n =69)和他汀对照组 (n =68) ;于发病 12~ 48小时和治疗 8周后分别测定血清基质金属蛋白酶 1(MMP 1)、基质金属蛋白酶 9(MMP 9)、基质金属蛋白酶抑制因子 1(TIMP 1)、MMP 9/TIMP 1、高敏C 反应蛋白 (hs CRP)水平。另设健康对照组 (n =60 )与之对照。结果 :①治疗前他汀治疗组和他汀对照组的血清MMP 1、MMP 9、MMP 9/TIMP 1、TIMP 1、hs CRP水平均较健康对照组明显增高 (P <0 0 1~ 0 0 0 1) ,与血脂水平不相关。②他汀治疗组经辛伐他汀治疗 8周后血清MMP 1、MMP 9、MMP 9/TIMP 1、hs CRP水平较治疗前均明显降低 (P <0 0 0 1) ,他汀对照组除血清hs CRP水平降低外 (P <0 0 5 ) ,其他各项指标均无变化。结论 :早期应用他汀治疗 ,可减少急性冠状动脉综合征患者的冠状动脉粥样斑块基质成分的降解和炎症反应 ,具有稳定斑块作用。

【Abstract】 Objective: To observe the influences of simvastation on serum matrix metalloproteinases-1(MMP-1),MMP-9,tissue inhibitor of metalloproteinases-1(TIMP-1),MMP-9/TIMP-1,and high sensitive C-reactive protein (hs-CRP) levels of patients with acute coronary syndrome (ACS).Methods: One hundred thirty seven patients with ACS were assigned to simvastatin group (20 mg/d of simvastatin for 8 weeks, n =69) or control group ( No lipid-lowering drugs were used, n =68) in a randomized,donble-blind,controlled study. Serum levels of MMP-1,MMP-9,TIMP-1,MMP-9/TIMP-1,and hs-CRP were detected before and after treatment.Results: ①Patients with ACS had higher levels of serum MMP-1,MMP-9,TIMP-1,MMP-9/TIMP-1 and hs-CRP,compared with those of the healthy controls ( p <0.001,p <0.05,p <0.01, n =60),which were not correlated with levels of serum lipids. ②After simvastatin treatment,the levels of serum MMP-1,MMP-9,MMP-9/TIMP-1,and hs-CRP were significantly decreased ( p <0.001,p <0.01). Conclusion: Simvastatin decreased the breakdown of matrix collagen,and reduced inflammatory reaction in the patients with ACS. It has certain effect on plaque stabilization.
