【摘要】 目的　探讨“一对一”全程陪伴责任制助产的临床效果。方法　 2 0 0 0年 1 1月至 2 0 0 2年 4月在传统助产模式下分娩的 3775例初产单胎头位产妇为第一阶段分析对象 ,2 0 0 2年 5月至 2 0 0 3年 1 0月实行“一对一”责任制助产后分娩的 4 0 1 3例初产单胎头位产妇为第二阶段分析对象。将两阶段阴道分娩率、剖宫产率、产程时间、产后出血率、胎儿窘迫和新生儿窒息率、产妇满意度进行比较。结果　第二阶段较第一阶段剖宫产率、产后出血率、胎儿窘迫和新生儿窒息率显著降低 (P <0 0 1 ) ;经阴道分娩的初产妇活跃期和第二产程时间比第一阶段显著缩短 (P <0 0 5 ) ;产妇满意度提高。结论　“一对一”全程陪伴责任制助产这种模式 ,有利于产科质量的提高 ,值得推广。更多还原

【Abstract】 Objective To explore the clinic result of “one to one” whole process companion system and job responsibility delivery.Methods A total of 3775 primiparae with occipital position single fetus from November 2000 to April 2002 who delivered under the traditional pattern were the first stage objects;another 4013 primiparae with occipital position single fetus from May 2002 to October 2003 who delivered under job responsibility delivery system as the second stage objects.The rate of vaginal delivery,cesarean section,postpartum hemorrhage,fetal distress and anoxia of newborn and the partuient satisfaction degree were compared between the two stages.Results The rate of cesarean section,postpartum hemorrhage,fetal distress and anoxia of newborn in the second stage group were significantly lower than those in the first stage group(P<0 01).The periods of active phase and the second birth stage in vaginal delivery primiparas were significantly shorter in the second group(P<0 05).The partuient satisfaction degree increased.Conclusion The pattern of “one to one” whole process companion system and job responsibility delivery is helpful to improve the obstetric level.更多还原
