早期舌癌浸润深度与颈淋巴结转移的关系
The relationship between tumor invasive depth and cervical lymph node metastasis in tongue cancer at early stages.
【摘要】 目的:观察早期舌癌浸润深度与颈淋巴结转移的关系。方法:收集行颈淋巴清扫术的早期(T1、T2期)舌癌病例54例,测量原发灶肿瘤浸润深度,同时行颈淋巴结的细胞角蛋白(Cytokeratin,CK)AE1/AE3免疫组化染色,观察颈淋巴结转移与肿瘤浸润深度等临床病理因素的关系。结果:舌癌肿瘤浸润深度为1.0-11.0mm,平均为5.2mm。54例病例中发生颈淋巴转移者22例(40.74%)。Fisher’s精确概率检验表明,早期舌癌颈淋巴结转移与肿瘤浸润深度及病理分级密切相关;单因素Logistic回归表明,早期舌癌淋巴结转移与肿瘤浸润深度及病理分级相关;多因素Logistic回归表明早期舌癌淋巴结转移仅与肿瘤浸润深度相关(P<0.01)。T1、T2期舌癌肿瘤浸润深度>4mm者,颈淋巴结转移率(60%)显著高于≤4mm者(5.3%)(P<0.01)。结论:肿瘤浸润深度对评估早期舌癌颈淋巴结转移有重要意义。
【Abstract】 PURPOSE:To study the relationship between tumor invasive depth and cervical lymph node metastasis in tongue cancer at early stages. METHODS: 54 patients with early stages (T1,T2 stages) squamous cell carcinoma of the tongue were selected. Tumor invasive depth was measured with ocular-micrometer and cytokeratin AE1/AE3 in specimens obtained from these patients was examined by immunohistoathology. The relationship between clinicopathologic parameters and lymph node metastasis were evaluated in these cases. RESULTS The tumor invasive depth ranged from 1.0 to 11 .0mm with a mean depth of 5.2mm. Of the 54 patients, cervical lymph nodes metastasis was detected in 22 cases (40.74%). Fisher’s exact test showed that cervical lymph nodes metastasis of early stages tongue cancer was highly correlated with tumor invasive depth and histologic grade. Binary logistic regression also reflected that in early stages tongue cancer lymph node metastasis was correlated with tumor invasive depth and histologic grade, but multinominal logistic regression only found correlation between tumor invasive depth and lymph node metastasis (P <0.01). In T1 and T2 stages, patients with tumor invasive depth>4mm had more lymph node metastasis (60%) than those with tumor invasive depth≤4mm(5.3%)(P <0.01). CONCLUSION: Tumor invasive depth has a high predictive value for cervical lymph node metastasis in tongue cancer at early stages.
